Join us in making a real difference in the health of our community.</em></p>\n<h2>Job Summary</h2>\n<p>The Social Care Navigator I (SCN) helps individuals and families access necessary social and healthcare services by connecting them with appropriate resources and support systems. They assess needs, provide referrals, and offer ongoing support to ensure individuals receive the care they require to thrive. Ideal candidates possess strong communication and interpersonal skills, a passion for helping others, and experience in social services or related fields.</p>\n<p>In Addition The Social Care Navigator I will focus on nutritional health. This includes identifying nutritional needs, providing education, and facilitating access to healthy food resources and related services.</p>\n<h2>Responsibilities</h2>\n<p>SCN Screening & Navigation</p>\n<p>· Responsible for outreaching and engaging with Medicaid members telephonically and in person to assess health-related social needs.</p>\n<p>· Conduct HRSN screening, and comprehensive navigation for referrals to social care services.</p>\n<p>· Use technology platforms to document client eligibility, outreach activities and case notes, outcomes of referrals, and other tasks as required.</p>\n<p>· Collaborate and communicate with team members, partner-based navigators/CHWs, and community partners to manage members with complex needs.</p>\n<p>· Connect individuals with appropriate community resources, including healthcare providers, social service agencies, and other relevant organizations.</p>\n<p>· Facilitate referrals to services, tracking progress, and ensuring that needs are addressed effectively.</p>\n<p>· Maintaining accurate records of interactions, referrals, and outcomes, often using data platforms and adhering to established protocols</p>\n<p>· Work closely with other professionals, community partners, and clients to ensure seamless service delivery and effective communication.</p>\n<p>· Other tasks assigned by the senior Director of NYREACH.</p>\n<p>Nutrition Education and counseling:</p>\n<p>· Manage a caseload of assigned clients and conduct outreach, provide enhanced services, and conduct workshops, seminars, one-one consultations and presentations on healthy eating habits, disease prevention, and nutrition topics.</p>\n<p>· Provide personalized nutrition education and counseling to individuals and groups, focusing on healthy eating habits, meal planning, and addressing specific dietary concerns.</p>\n<p>· Offer clear and accurate information about healthy eating, portion control, food preparation, and the impact of nutrition on overall health.</p>\n<h2>Qualifications</h2>\n<p>Bachelor Degree in Nutrition, Public health with concentration in nutrition, Dietetics, Food science, clinical nutrition, community nutrition or related field.</p>\n<p>· Registered dietician (or eligible to sit for the RDN Exam)</p>\n<p>· Strong communication and organizational skills.</p>\n<p>· Effective verbal and written communication is essential for interacting with clients, providers, and other stakeholders.</p>\n<p>· Cultural sensitivity and the ability to adapt to different needs and work with a diverse population.</p>\n<p>· Bilingual Spanish required</p>\n<p> </p>\n<p><strong>Work Environment:</strong></p>\n<p>· Onsite, full-time position (Monday–Friday).</p>\n<p>· Flexible setting that includes in-office, community events, Community organizations and partners, health care facilities etc.</p>\n<p>· Direct work with clients in person, over the phone and through other communication methods.</p>\n<p> </p>\n<p>$24-$25 an hr</p>\n<h2>Equal Opportunity Employer</h2>\n<p><strong><em>Essen Health care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population.</em></strong></p>",
