Your key responsibilities will include: Medical Advocacy & Accompaniment: Provide occasional in-person advocacy, support, and medical care coordination for clients during their appointments at local doctors' offices. Clinical Mentorship & Support: Serve as a primary medical resource for our BHH Care Coordinators and Clinical Coordinators, interweaving physical health strategies into behavioral health care plans. Consultative Client Care: Provide remote and in-person consultative support for clients transitioning out of hospitals, emergency rooms, or treatment facilities to ensure safe and successful discharges. Health Education & Training: Step into a leadership role by conducting health education sessions for clients and offering vital health-literacy guidance to our non-medical staff. Data-Driven Coordination: Utilize our Electronic Health Record (EHR) to track client progress, analyze health outcomes, and translate medical data into actionable interdisciplinary care plans. Community Outreach: Advocate for client access to crucial external resources and coordinate seamlessly with external community health providers. Safety & Operational Advisory: Advise AWS management on health, safety, and infectious disease protocols, assisting in the development of robust internal policies.
