Description This position’s primary goal is to prevent communicable disease in the community. This position is responsible for educating community partners regarding the administration of vaccinations to children and teens. Nursing activities include, but are not limited to: systematic analysis of health data; health education and advocacy; coalition building; and collaborating with community partners to promote the health of a population. This position will strive to: build healthy communities through promotion of healthy environments; achieve health equity and social justice; improve quality of life and increase access to care. This position may be involved with special health projects in the community including, but not limited to, the Community Health Assessment, nursing interventions, and goal setting with a client and family. Responsibilities Trains and communicates with providers, schools, and other agencies regarding vaccination schedules and best practices. Using data obtained from the State of Ohio GVO program, educates school nurses and medical providers on the best practices to raise vaccination rates. Works with providers to develop quality improvement plans regarding vaccination rates. Generates reports for GVO grant deliverables. Prepares and maintains client records through the entry of accurate and appropriate patient information into the electronic health records systems. Serves as a resource for the community regarding vaccinations. Serves as a back- up for the Shots for Tots Nurse. Reviews client shot records. Gives vaccinations. Completes shot record requests. Ensures that the clinic is stocked with supplies and vaccinations. Checks inventories to ensure that supplies are not expired and safe to use with patients. Monitors refrigerator temperatures to vaccine quality assurance. Tracks and updates perinatal hepatitis B investigations. Updates school contact information. Conducts vaccination clinics off-site. Cleans clinic area once patients are seen. Charts the vaccinations in the EMR; completes follow-ups and sends post cards to patients to remind individuals that have missed vaccinations to get them. Compiles information and writes reports as needed. Audits charts to ensure that the best nursing practices are being adhered to. Perform outreach functions, attend interagency meetings as assigned, and work collaboratively with all community agencies. Perform initial client assessment by acquiring health and family histories, conduct routine medical tests, and make appropriate referrals. Counsels clients and provides health education in areas of parenting, child development, health and medical problems and services, and life cycle tasks for the purpose of improving or enhancing the personal health of the client(s). Adheres to all Federal, State and Local laws, regulations, rules and/or guidelines including HIPAA regulations as well as all grant requirements, where applicable. Evaluates outcomes of public health nursing interventions; work with others (clients and other professionals) to make changes as necessary. Assists with developing, preparing, and coordinating grant applications and grant-related activities to obtain funding for program and related work. Mentors students as public health leaders to strengthen the public health nursing workforce. Promotes evidence-based practice, maintain best practices, and determine and maintain essential Public Health Nurse Competencies. Assists and works with other nursing staff as needed. This includes, but is not limited to, covering other nursing areas such as Adult Travel Immunizations, TB, etc. Adheres to PHAB and Quad Council Coalition nursing core competencies including, but not limited to: 1A1, 1A4, 1A7, 1A10, 1A11, 2A4a, 2A8, 3A2, 3A6, 4A3, 5A1a, 6A1, 6A3, and 8A1. Participates in quality improvement efforts and achieving PHAB accreditation requirements. This position will be responsible for using quality improvement (QI) processes and/or techniques to improve the effectiveness of the assigned public health program. Attends and participates in conferences, seminars, trainings, or related education classes as assigned. The essential duties and responsibilities defined within this position description are not an all-inclusive list, but a general summary of purpose and primary function of the position. An individual employed in this position may be asked or assigned to perform a wide-range of related tasks, within the scope of the position, department/division, union affiliation, etc. as deemed appropriate by management. Qualifications ADN and 2 years of nursing experience Registered Nursing license in the state of Ohio Holds Basic Life Support (BLS) certification for health care providers. Ability to utilize electronic medical software Ability to utilize ODRS Ability to utilize MS Word and MS Excel Strong computer skills Knowledge of modern medical procedures and protocols Ability to work evenings & weekends during busy season Must have and maintain throughout employment a valid driver’s license, reliable transportation, and be insurable under the Lucas County Commissioners Risk Management. This position is classified as Clinic - 2 setting and performs a wide range of functions for the Toledo-Lucas County Health Department. Further information may be obtained from Human Resources. Organization The Toledo-Lucas County Health Department (TLCHD) is accredited by the Public Health Accreditation Board and serves as the local health authority for Lucas County. It operates as a regional health district, serving all cities, townships, and villages within the county.
