Embodies the Memorial Health System Performance Excellence Standards of Safety, Courtesy, Quality, and Efficiency that support our mission, vision and values.</p>\n<p> </p>\n<p><strong>To review Memorial's Benefits click here: Benefits - Memorial HR</strong></p>\n<h2>Qualifications</h2>\n<p><em>Education:</em></p>\n<ul>\n <li>Bachelor’s Degree in Dietetics required. </li>\n</ul>\n<p> </p>\n<p><em>Licensure/Certification/Registry:</em></p>\n<ul>\n <li>Registration with commission on Dietetic Registration, Licensure with the state of Illinois required.</li>\n</ul>\n<p> </p>\n<p><em>Experience:</em></p>\n<ul>\n <li>Completion of approved internship preferred. </li>\n</ul>\n<p> </p>\n<p><em>Other Knowledge/Skills/Abilities/Working Conditions-Physical Requirements & Atmosphere:</em></p>\n<ul>\n <li>Computer literacy with Windows based systems.</li>\n <li>Communicate effectively.</li>\n <li>Work independently, making independent decisions within departmental guidelines</li>\n <li>Work under pressure in a noisy and hot environment and accomplish multiple tasks.</li>\n <li>Be flexible with scheduled work hours and assignment.</li>\n <li>Physically able to walk, stand and lift 20#’s.</li>\n <li>Good manual dexterity.</li>\n <li>Visual and auditory acuity.</li>\n <li>Excellent customer service skills.</li>\n <li>May be exposed to blood and body fluids.</li>\n</ul>\n<h2>Responsibilities</h2>\n<ul>\n <li>Assesses patients’ nutritional status according to established standards.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Obtains necessary information from the medical record to determine patient’s nutritional status</li>\n</ul>\n<p> </p>\n<ul>\n <li>Develops and documents nutritional care plans for patients of any age including neonate, child, adolescent, geriatric and terminally ill patients</li>\n</ul>\n<p> </p>\n<ul>\n <li>Develops and documents nutritional care plans based on the cultural orientation of the patient.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Embodies the Memorial Health System Performance Excellence Standards of Safety, Courtesy, Quality, and Efficiency that support our mission, vision and values:</li>\n</ul>\n<p> </p>\n<ul>\n <li>\n  <ul>\n   <li>\n    <ul>\n     <li><strong><u>SAFETY:</u></strong> <em>Prevent Harm</em> - I put safety first in everything I do. Provides nutritional teaching and counseling to patient/family in making desired changes in eating habits.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Provide nutritional assessment and follow-up for nutritional support patients</li>\n</ul>\n<p> </p>\n<ul>\n <li>Screen inpatients not identified at risk when hospitalized for more than 7 days</li>\n</ul>\n<p> </p>\n<ul>\n <li>Collect performance improvement data.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Provide training for diet office staff.</li>\n</ul>\n<p> </p>\n<ul>\n <li>Serve as preceptor for dietetics students</li>\n</ul>\n<p> </p>\n<ul>\n <li>Performs other related work as required or requested.</li>\n</ul>\n<p><em> </em></p>\n<p><em> </em></p>\n<p><em> </em></p>\n<p><em>The intent of this job description is to provide a representative summary of the major duties and responsibilities performed by incumbents of this job.
