Licensed or license pending in the State of Illinois without previous disciplinary action in Illinois or other states. Current CPR card. Responsible for providing and coordinating comprehensive care to patients of all ages through the proper utilization of evaluation, planning and implementation of a treatment plan in accordance with established standards, policies, procedures, protocols and guidelines. Embodies the Memorial Health System Performance Excellence Standards of Safety, Courtesy, Quality, and Efficiency that support our mission, vision and values. The maximum allowable time to practice in a license pending status/under a licensed Occupational Therapist is 6 months per licensing regulations. CPR Certification preferred. If applicable, must possess valid Illinois driver’s license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (5 year MVR will be required). Experience: Experience with neurologic and orthopedic injuries desirable. Responsibilities Other Knowledge/Skills/Abilities: Basic Knowledge in occupational therapy practices in accordance with O.T. Practice Act. Universal Precautions Category I. Must possess valid Illinois driver’s license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (five year MVR will be required). Must show proof of automobile insurance. Provides rehabilitative care to patients of all ages. Evaluates treatment needs of assigned physician referred patients of all ages. Determines and implements plan to accomplish treatment goals within appropriate time frames. Provides appropriate feedback to physician, department, and other team members and maintains required documentation. Embodies the Memorial Health System Performance Excellence Standards of Safety, Courtesy, Quality, and Efficiency that support our mission, vision and values: SAFETY: Prevent Harm - I put safety first in everything I do. I take action to ensure the safety of others. COURTESY: Serve Others - I treat others with dignity and respect. I project a professional image and positive attitude.
