    "description": "<h2>Job Summary</h2>\n<h2>Medical Records Technician (Coder) – Inpatient Facility (ICD-10-PCS)</h2>\n<p><strong>Location/Hours:</strong> Remote - Monday–Friday, 8:00am–4:30pm CT<strong>Client:</strong> Central Texas Veterans Health Care System (CTVHCS) – Temple, TX</p>\n<p><strong>Compensation: $</strong>22.47/hr plus $5.09 for Health and Wellness</p>\n<h3>Role Summary</h3>\n<p>Responsible for accurate, timely facility inpatient coding and data validation across a broad range of specialties. Reviews medical records for complete documentation, assigns and sequences ICD-10-CM/PCS and related codes, and queries providers for clarification using VA tools.</p>\n<h2>Primary Responsibilities</h2>\n<h3>Key Responsibilities</h3>\n<ul>\n <li><p>Perform <strong>facility inpatient coding</strong> including <strong>ICD-10-CM, ICD-10-PCS, DRGs, and HCPCS</strong> as assigned.</p></li>\n <li><p>Review full medical record for principal diagnosis/procedures, complications/co-morbidities, and documentation adequacy.</p></li>\n <li><p>Ensure coding supports accurate billing and clinical/statistical reporting.</p></li>\n <li><p>Query clinicians for documentation clarification using <strong>VIRR</strong> (approved query format); close queries timely per requirements.</p></li>\n <li><p>Maintain <strong>100% data validation</strong> for assigned encounters.</p></li>\n <li><p>Collaborate with claims/billing staff on documentation and coding/billing issues as needed.</p></li>\n <li><p>Support audits, corrective actions, and training efforts in coordination with the auditor and COR.</p></li>\n</ul>\n<h3>Performance Standards (high level)</h3>\n<ul>\n <li><p>Code records within <strong>7 calendar days</strong> (<strong>>95% compliance</strong>).</p></li>\n <li><p>Maintain <strong>≥95% accuracy</strong> for ICD-10-CM/PCS and related coding.</p></li>\n <li><p>Provide monthly QA/improvement reports and other ad hoc reporting as directed.</p></li>\n</ul>\n<h2>Minimum Qualifications</h2>\n<h3>Required Qualifications</h3>\n<ul>\n <li><p>Active certification: <strong>RHIT, RHIA, CCS, CCS-P, and/or CPC</strong> (must maintain throughout contract).</p></li>\n <li><p><strong>3+ years continuous coding experience</strong> in a comparable or larger facility setting.</p></li>\n <li><p>Proficiency with <strong>ICD-10-CM, ICD-10-PCS, DRGs, HCPCS</strong> and facility coding workflows.</p></li>\n <li><p><strong>U.S. citizen</strong>, proficient in written/spoken English.</p></li>\n <li><p>Ability to work within VA systems (e.g., <strong>VistA/CPRS, VIRR</strong>) and comply with HIPAA/VA directives.</p></li>\n</ul>\n<h2>About Aptive</h2>\n<p>Arrow ARC supports Veterans Health Administration facilities and offices across the U.S. with health care staffing and program support via the 10-year Integrated Critical Staffing Program (ICSP).
