This role establishes individualized physical therapy goals, provides treatments, and regularly reassesses patient progress, documenting all findings accurately.&nbsp;<br><br><strong>Essential Functions</strong></p><ul><li>Conducts thorough physical therapy evaluations to assess patient needs and develops individualized treatment plans.</li><li>Administers safe and effective therapy treatments, ensuring all interventions are appropriately documented.</li><li>Reassesses and adjusts treatment plans within appropriate timeframes to align with patient progress and changing needs.</li><li>Provides clear, concise, and accurate documentation of evaluations, treatment plans, progress notes, and billing information.</li><li>Educates patients and their families on treatment plans, therapeutic exercises, and post-treatment care to support long-term recovery.</li><li>Operates therapy equipment safely and maintains a clean, organized, and safe environment for patients and staff.</li><li>Collaborates with referring physicians, peers, and interdisciplinary team members to ensure continuity and quality of patient care.</li><li>Demonstrates population-appropriate competencies for all relevant patient demographics, tailoring care to individual needs.</li><li>Maintains compliance with facility policies, including safety, infection control, and risk management procedures.</li><li>Adapts to changes professionally and effectively, following the chain of command to address and resolve issues impacting patient care.</li><li>Performs other duties as assigned.</li><li>Maintains regular and reliable attendance.</li><li>Complies with all policies and standards.</li></ul><p><strong>Qualifications</strong></p><ul><li>Doctorate in Physical Therapy (DPT) required</li><li>0-2 years of experience in a physical therapy setting required</li></ul><p><strong>Knowledge, Skills and Abilities</strong></p><ul><li>Strong knowledge of physical therapy principles, therapeutic techniques, and safety standards.</li><li>Ability to perform comprehensive evaluations and develop effective treatment plans.</li><li>Proficiency in using therapy equipment and technology for documentation and billing.</li><li>Excellent communication and interpersonal skills to collaborate with patients, families, and interdisciplinary teams.</li><li>Strong organizational and time management skills to prioritize tasks and maximize productivity.</li><li>Commitment to professional development and adherence to ethical and legal standards.</li></ul><p><strong>Licenses and Certifications</strong></p><ul><li>PT - Physical Therapist license in the state of employment required</li><li>BCLS - Basic Life Support within 30 days of hire required</li></ul>",
