Conduct clinical consultations and guide admission decisions for prospective residents Coordinate and oversee incoming patient referrals and admissions Support census management by ensuring appropriate clinical placement across service lines based on patient needs Collaborate with hospital, clinicians, discharge planners, families, and referral partners to support continuity of care and timely admissions Review referrals for medical appropriateness and coordinate admissions Manage clinical intake, including records review, insurance verification, and clinical transitions Maintain positive relationships with referral sources and promote Maple Crest programs and services What you will need:
Active Nebraska RN License (unencumbered) 2 years of skilled nurse experience (required) Knowledge of Medicare, Medicaid, managed care, and skilled nursing admission processes Strong critical thinking skills, independent judgment, and the ability to multitask and prioritize work assignments in a fast-paced work environment Ability to understand, read, write, and speak English Benefits for Qualified Employees Medical Dental Vision 403(b) with Discretionary Employer Match Short- & Long-Term Disability Accident & Critical Illness Insurance Employee Assistance Program To apply, please complete the required questionnaire.
