font-size: 16px; color: rgb(0, 0, 0);\">Under the direction and guidance of the Advanced Practice Provider:&nbsp;</span></p><ul><li style=\"color: rgb(0, 0, 0);\"><span style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Facilitates efficient and effective patient flow including preparing the patient for the visit/admission, and providing specific services and education as directed by advanced practice provider&nbsp;<br></span></li><li style=\"color: rgb(0, 0, 0);\"><span style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Rooms patients and collects pertinent clinical information inclusive appropriate screenings, measuring vital signs, such as pulse rate, temperature, blood pressure, weight, and height, and records information in patient chart&nbsp;<br></span></li><li style=\"color: rgb(0, 0, 0);\"><span style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Performs necessary administrative duties including maintenance and monitoring of EHR inboxes&nbsp;<br></span></li><li style=\"color: rgb(0, 0, 0);\"><span style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Operates electrocardiograph (EKG) and other equipment to administer routine diagnostic test as directed&nbsp;<br></span></li><li style=\"color: rgb(0, 0, 0);\"><span style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Coordinates referrals and prior authorizations for patient care&nbsp;<br></span></li><li style=\"color: rgb(0, 0, 0);\"><span style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Accurately documents patient information in EHR from various sources including phone calls, patient messaging, remote patient monitoring technologies&nbsp;<br></span></li><li style=\"color: rgb(0, 0, 0);\"><span style=\"font-family: tahoma, sans-serif; font-size: 16px;\">Reviews appointment schedules in advance and prepares charts for upcoming appointments inclusive of reviewing medical records, lab and diagnostic results, chronic conditions, and outstanding quality gaps to ensure chart is prepared for provider visit&nbsp;<br></span></li><li style=\"color: rgb(0, 0, 0);\"><span style=\"font-family: tahoma, sans-serif;
