Employees in this role are cross‑trained and scheduled in each program based on organizational needs. They uphold CCARC&rsquo;s mission by ensuring consistency, safety, and high‑quality support across both environments.</p><p><strong>Primary Responsibilities</strong></p><p><strong>Day Program Responsibilities</strong></p><ul type=\"disc\"><li>Support individuals in daily activities, skill development, community outings, and vocational tasks.</li><li>Maintain accurate documentation, including daily notes, goals, attendance, and progress tracking.</li><li>Assist with personal care as needed, following program protocols.</li><li>Encourage independence, engagement, and community inclusion throughout the day.</li></ul><p><strong>Residential Program Responsibilities</strong></p><ul type=\"disc\"><li>Provide evening, morning, and weekend support within assigned residences.</li><li>Assist individuals with ADLs (Activities of Daily Living), household routines, medication administration (if certified, if not certified, you will be required to become certified within 6 months of hire), and community engagement.</li><li>Maintain safety, cleanliness, and nurturing home environments.</li><li>Complete shift documentation, communication logs, and incident reporting as required.</li></ul><p><strong>Cross‑Program Expectations</strong></p><ul type=\"disc\"><li>Attend training required to maintain competency across both program areas.</li><li>Communicate effectively with both Day and Residential supervisors regarding scheduling, priorities, and individual needs.</li><li>Ensure consistency in individual care plans, behavior strategies, and support approaches across settings.</li><li>Demonstrate flexibility and professionalism when transitioning between program environments.</li></ul><p><strong>Skills &amp; Competencies</strong></p><ul type=\"disc\"><li>Strong communication and teamwork across both departments</li><li>Time management and ability to adapt to changing needs</li><li>Reliable, consistent, and capable of working independently or within a team</li><li>Knowledge of individual support plans, safety protocols, and agency policies</li></ul><p><strong>Schedule &amp; Supervision</strong></p><ul type=\"disc\"><li>Hybrid DSPs are scheduled in both the Day Program (typically weekday daytime hours) and Residential settings (mornings, evenings, and/or weekends).</li><li>Supervision is shared between the Day Program Manager and Residential Manager, who coordinate training, expectations, and performance feedback.</li></ul></div></div></div></div></div></div></div></div>\n<br/>Hybrid- PLAN / CES<br/>PLAN009<br/>Thursday 8a - 8p; Friday 8a - 8p; Saturday 10a - 6p<br/>FT 32 HOURS ",
