Working within the framework of hospital guidelines, protocols, and the unit's care delivery approach, the CCA executes tasks under the supervision of the Registered Nurse, ensuring patients' well-being and comfort.</p><p><strong>Duties</strong></p><ul><li>Follows KARDEX for patient care needs, including communication, meal assistance, transferring, ambulation, toileting, hygiene, and grooming.</li><li>Executes individualized care plans, addressing instructions such as diet restrictions, safe transferring, pressure ulcer prevention, restraint use, fall prevention, and assisted communication.</li><li>Reports patient condition changes to the Registered Nurse promptly and objectively, including skin integrity, mental status, sleep patterns, nutritional factors, weight, bowel functions, and behaviors.</li><li>Assists with psychosocial patient needs, responds to call bells, answers phones, supports healthcare team, provides coverage during breaks, attends meetings, and contributes to a safe environment.</li><li>Documents care and changes in patient condition following hospital policy, using Core Care Summary and Progress Notes.</li><li>Operates equipment like lifts and wheelchairs, ensuring functionality and notifying staff of concerns.</li><li>Maintains a safe environment by adhering to safety standards, identifying and preventing safety issues, attending safety meetings, and using personal protective equipment.</li><li>Exhibits the Center's core values and contributes to patients' well-being.</li></ul>",
