This role involves assisting pharmacists in managing medication therapy, maintaining patient records, and ensuring safe and effective medication use&mdash;while also helping patients access affordable medications through specialized programs.</p><p style=\"margin-left:0in;\"><strong>&nbsp;</strong></p></td></tr><tr><td valign=\"top\" style=\"width: 100%;\"><p style=\"margin-left:0in;\"><strong>Essential Duties and Responsibilities:&nbsp;</strong></p><ul type=\"disc\"><li>Function as support and resource for the Clinical pharmacists including but not limited to TCM, MTM, medication reconciliation, CMS and third party payer benchmarks, and&nbsp;patient medication adherence assistance.&nbsp;</li><li>Perform the basic functions for TCM including, but not limited to, contacting patients for status check, obtaining records, managing appointments, assisting with medication reconciliation, and record development/reporting.</li><li>Conduct medication reconciliation by reviewing patient records</li><li>Support clinical interventions and medication therapy reviews</li><li>Communicate with healthcare providers and patients regarding prescriptions</li><li>Ensure compliance with safety protocols and regulatory standards</li><li>Assist patients in accessing medications with high cost or limited insurance coverage through various programs, such as:<ul type=\"circle\"><li>340B Drug Pricing Program</li><li>Sliding Fee Discount Programs</li><li>Patient Assistance Programs (PAPs)</li></ul></li><li>Verify patient eligibility for PAPs and gather required documentation for applications</li><li>Follow up on pending PAP applications and communicate status updates to patients</li><li>Monitor and manage PAP medications, including completing paperwork to request manufacturer refills</li><li>Maintain accurate records of PAP inventory and ensure timely renewals</li><li>Support retail pharmacy space as needed (see pharm tech job description)&nbsp;</li><li>Maintain compliance with all HIPAA and privacy standards</li><li>Maintain compliance with all company policies and procedures</li><li>Other duties as assigned.</li></ul></td></tr><tr><td valign=\"top\" style=\"width: 100%;\"><p style=\"margin-left:0in;\"><strong>Education and/or Work Experience Requirements:&nbsp;</strong></p><p style=\"margin-left:0in;\"><strong>Education &amp;
