If this is you, keep on reading.</p>\r\n\r\n<p><strong>Your Goal:</strong></p>\r\n\r\n<p>The Medical Case Manager provides&nbsp;medical case management services for people living with HIV/AIDS.</p>\r\n\r\n<p><strong>How You&#39;ll Help Patients:</strong></p>\r\n\r\n<ul>\r\n\t<li>Compete initial eligibility assessment.</li>\r\n\t<li>Development of comprehensive, individualized care plan jointly with the client/patient that includes short- and long-term goals focused on attaining, maintaining, and achieving positive health outcomes.</li>\r\n\t<li>Facilitate timely and coordinated access to medically appropriate levels of health and support services and continuity of care.</li>\r\n\t<li>Provide continuous client/patient monitoring to assess the efficacy of the individualized care plan.</li>\r\n\t<li>Coordinate the individualized care plan with client&rsquo;s/patient&rsquo;s primary care provider.</li>\r\n\t<li>Refer and link client/patient to appropriate services within the system of care that promote positive health outcomes, treatment adherence, and greater self-sufficiency. Monitor the client&rsquo;s follow through with these services.</li>\r\n\t<li>Enter and utilize the client/patient level database for the tracking and reporting of all services provided.</li>\r\n\t<li>Follow the Standard Operating Procedures of the AIDS Foundation of Chicago&rsquo;s Northeastern Illinois HIV/AIDS Case Management Collaborative.</li>\r\n\t<li>Engage in all offered training, consultations and supervisions made available.
