The Peer Specialist will provide support and linkage within 24 hours per day, 7 day per week behavioral health Crisis Stabilization Center with a 24/7 Hotline and Mobile Crisis Team.</p><p><strong>&nbsp;</strong></p><p><strong>RESPONSIBILITIES</strong></p><ul><li>The Peer Specialist will provide outreach to clients and/or their families in the field, by phone, and in person.</li><li>Facilitate engagement in services and offer linkage to community resources.</li><li>Participate in Crisis Assessments and linkages as part of multidisciplinary team.</li><li>Provide recovery readiness support through linkage to community resources.</li><li>Provide warm hands off to all levels of treatment.</li><li>Provide person centered care and share lived experience to facilitate engagement.</li><li>Coordinate with collateral contacts, including family and other providers.</li><li>Utilize motivational interviewing to assist individuals in readiness for change.</li><li>The Peer Specialist will provide opioid overdose education and train individuals and family to use Narcan.</li><li>Work as part of a multidisciplinary team, partnering with Social Workers, Registered Nurses, Nurse Practitioners and Peers to work collaboratively with clients needing assessments and linkage.</li><li>Coordinate care planning with other providers of services/resources to ensure goal directed, collaborative care, including care transitions.</li><li>The Peer Specialist will act as a resource to all team members on community-based recovery supports.</li><li>Complete thorough and timely documentation according to regulations in an Electronic Health Record.</li><li>Complete trainings and maintain proficiency with de-escalation and crisis intervention techniques.</li><li>Conduct field work.</li><li style=\"margin-left:0in;\" data-pasted=\"true\">This position is essential to the safe and continuous operation of the program. Essential employees are expected to report to work during inclement weather and other relevant emergency situations to ensure uninterrupted care and support for clients.</li><li>All other duties as assigned.</li></ul><p><strong>&nbsp;</strong></p><p><strong>QUALIFICATIONS</strong></p><p>High School diploma or equivalent required. &nbsp;</p><p>Peer Certification or willingness to become certified required.</p><p>Prior experience as a peer, paid or volunteer preferred.&nbsp;</p><p>Relevant life experience required and willingness to draw upon this experience to engage individuals and serve as a role model.</p><p>Excellent interpersonal and verbal and written communication skills required.</p><p>Ability to work both independently, as well as part of a team. Must be responsive to time deadlines and able to multitask.</p><p>Proficient computer skills, including Microsoft Office required; experience with EHR preferred.</p><p>Valid and clean NYS Driver&rsquo;s License required.&nbsp;</p><p>Possible evening and weekend hours.&nbsp;</p><p><strong>&nbsp;</strong></p><p><strong>PHYSICAL REQUIREMENTS&nbsp;</strong></p><p>Ability to bend, stretch and stand for long periods of time and the ability to physically restrain clients.</p><p>Ability to safely operate a motor vehicle for the Mobile Response Team.</p><p>General computing and keyboard skills.</p></div></div>\n<br/>One or more of the following shifts required:<br/><br/>Sunday 8a-4p<br/>Sunday 4p-12a<br/>Friday 4p-12a",
