          "text": "<p>Work collaboratively with our established multidisciplinary Seniors’ Clinic team to provide comprehensive geriatric care to patients aged 65 years and older, with a particular focus on frail older adults.&#xa0;</p><p><strong>Responsibilities</strong></p><ul><li>Operate within the <a href=\"https://nshdocs.morethanmedicine.ca/practicing-medicine-nova-scotia/immigration-compliance-resources\">Bylaws and Rules and Regulations </a>of Nova Scotia Health Authority.&#xa0;</li><li>Services will be delivered through consultation in the following areas:<ul><li>Inpatient Geriatrics and Pre-operative Geriatric Optimization at the Colchester East Hants Health Centre (Truro, NS).</li><li>Outpatient, office-based geriatric consultations for community-dwelling older adults at the Rath Eastlink Community Centre (Truro, NS), provided in person and/or virtually. This includes case reviews and collaborative support with Geriatric clinicians and at times, when appropriate, patient home visits</li><li>Consultative services to long-term care facilities throughout Colchester County.</li></ul></li><li>The candidate will also participate in a shared on-call schedule, providing telephone consultation to primary care providers managing older adults in community, long-term care, and acute care settings within the Northern Zone.
