    "description": "<p data-olk-copy-source=\"MessageBody\">Program: START</p>\r\n\r\n<p>Location: Hartford</p>\r\n\r\n<p>Salary: $22.00/hour</p>\r\n\r\n<p>Schedule: Full time; 40 hours; 1st shift; Monday-Friday 8 am-4pm.&nbsp;</p>\r\n\r\n<p aria-hidden=\"true\">&nbsp;</p>\r\n\r\n<p>The Start Program is a nationally recognized program that works with homeless and unstably-housed youth, aged 18-24, located throughout the state of CT through an innovative service delivery model that combines short-term rental assistance with intensive, community-based case management services targeted towards housing stabilization.&nbsp; Through a lens of trauma-informed care and positive youth development, the program partners with young adult participants to identify goals and resources that help them to exit homelessness and achieve long-term stability.</p>\r\n\r\n<p>The Case Manager 2 is responsible for providing community-based case management services at the agency-leased shelter to homeless young adults located in Hartford. Responsibilities include;</p>\r\n\r\n<ul type=\"disc\">\r\n\t<li>delivering comprehensive case management services to clients in their homes and in their communities,</li>\r\n\t<li>conducting intake assessments,</li>\r\n\t<li>assisting youth in locating available apartments for rent,</li>\r\n\t<li>working in partnership with youth to develop and implement individual service plans that will progress them to self-sufficiency,</li>\r\n\t<li>liaison with treatment providers and community resources,</li>\r\n\t<li>providing transportation to clients as needed,</li>\r\n\t<li>developing money management skills with caseload,</li>\r\n\t<li>developing employment readiness skills with caseload,</li>\r\n\t<li>liaison with landlords and property management staff to support youth&#39;s continued tenancy,</li>\r\n\t<li>data entry and documentation,</li>\r\n\t<li>caseload size will vary between 6-10&nbsp;young adults at any given time</li>\r\n</ul>\r\n\r\n<p>Working within our Young Adult Services, the Case Manager 2 works with the START program staff to provide services to stabilize and reunite families experiencing complex issues such as substance use, behavioral health matters, and homelessness.&nbsp;</p>\r\n\r\n<p><u>Requirements:</u></p>\r\n\r\n<ul type=\"disc\">\r\n\t<li>Associate&rsquo;s Degree</li>\r\n\t<li>2+ years relevant experience in direct service and case management field</li>\r\n\t<li>Valid Connecticut Driver&rsquo;s License</li>\r\n\t<li>Regular and predictable attendance is required</li>\r\n</ul>\r\n\r\n<p><u>Orientation:</u></p>\r\n\r\n<ul type=\"disc\">\r\n\t<li>If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.</li>\r\n</ul>\r\n\r\n<p><u>Benefits:</u></p>\r\n\r\n<ul type=\"disc\">\r\n\t<li>Medical, dental, vision benefits are offered along with other additional voluntary coverages with full time employment</li>\r\n\t<li>403(b) retirement plan with employer matching contribution</li>\r\n\t<li>Company paid short and long term disability and life insurance with full time employment</li>\r\n\t<li>Paid time off (vacation, personal and sick) with full time employment</li>\r\n\t<li>12 paid holidays</li>\r\n</ul>\r\n\r\n<p><i>The Connection is a statewide human services and community development agency that provides unique solutions to the problems of homelessness, mental illness, substance use, improving child welfare and community justice rehabilitation.
