Monitor individualized treatment and rehabilitation plans. Provide direct services as assigned.</p><p><strong>Essential Responsibilities</strong>:&nbsp;</p><ol><li>Provide coordination and linkage of services, advocacy and direct care as needed to individuals and/or family for the purpose of overall life management.</li><li>Work with Program Manager to ensure compliance with BHDDH &amp; CARF regulations.</li><li>Schedule, attend and participate in mandated center and team meetings.</li><li>Facilitate and monitor effective delivery of services to identified program.</li><li>Conduct supervisory spot checks of all mandated documentation to ensure compliance.</li><li>Complete monthly reports in accordance with center policies.</li><li>Provide new staff with program orientation and ongoing professional development and training.</li><li>Provide support and training to staff&#39;s assigned caseload.</li><li>Provide medication monitoring and support.</li><li>Ensure scheduling of program staff meets BHDDH regulations.</li><li>Ensure group home is operating smoothly.</li><li>Support symptom identification and management through identifying symptoms of decompensating, identifying coping strategies and resources to deal with symptoms of mental illness, and educating members regarding their diagnosis and symptoms.</li></ol><p><strong>Requirements:</strong></p><ul><li>Bachelor&rsquo;s Degree in the Human Service field or an equivalent combination of education and experience required.&nbsp;</li><li>Minimum (2) year experience in Human Service field working with clients with disabilities preferred</li><li>Training in related state and federal assistance programs also preferred</li><li>Basic computer literacy skills are necessary</li><li>Valid driver&#39;s license and automobile insurance which meets Rhode Island minimum standards required</li></ul><p><strong>Benefits:</strong></p><p>Thrive Behavioral Health provides a friendly/family working environment, whose values include Dignity, Respect, Teamwork and Professional Support and Development.
