This individual should have balanced independence and a collaborative mindset.</p>\n<p><strong>Qualified applicants should possess:</strong></p>\n<ul>\n <li>HS Diploma</li>\n <li>Working knowledge of Microsoft Office Programs</li>\n <li>Prior work experience in the healthcare field is preferred</li>\n</ul>\n<p><strong>Benefits:</strong></p>\n<ul>\n <li>Comprehensive Benefit Package</li>\n <li>Paid Time Off</li>\n <li>Paid Holidays</li>\n <li>401K + Match</li>\n</ul>\n<p> </p>\n<p> </p>\n<p><strong>PURPOSE STATEMENT:</strong></p>\n<p>Manage the client's/patient's/resident’s cases with a solid background of clinical knowledge and expertise and may be responsible for providing some therapeutic services to clients/patients/residents.</p>\n<h2>Responsibilities</h2>\n<p>ESSENTIAL FUNCTIONS:</p>\n<p>· Provide case management and care coordination services for clients/patients/residents in the facility.</p>\n<p>· Responsible for coordinating continuum of care activities for assigned clients and ensuring optimum utilization of resources, service delivery, and compliance with external agencies and referral sources requirements.</p>\n<p>· Responsible for psychosocial assessment and for conducting individual, family and group sessions.</p>\n<p>· Conduct various duties related to coordinating treatment and discharge activities, as needed.</p>\n<p>· Provide quality care through developing, implementing, managing and evaluating client/patient care plans.</p>\n<p>· Collect, correlate and provide clinical data to the treatment team.</p>\n<p>· Convey medical criteria and clinical information between the insurance provider and treatment team as warranted.</p>\n<p>· May also correlate clinical data for business office as required.</p>\n<p>· Coordinate in a timely manner, issues or activities relevant to the treatment team.</p>\n<h2>Qualifications</h2>\n<p>EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:</p>\n<p>· Bachelor’s degree in Social Work, Counseling or related human service field required.</p>\n<p>· One or more years’ experience working in a behavioral health setting preferred.</p>\n<p>· Previous experience with the patient population served by the facility preferred.</p>\n<p>· Previous experience in case management and/or utilization review preferred.</p>\n<p> </p>\n<p> </p>\n<p>LICENSES/DESIGNATIONS/CERTIFICATIONS:</p>\n<p>· Current license required within the state the facility is operating in if required.</p>\n<p>· CPR and de-escalation/restraint certification required (training available upon hire and offered by facility).</p>\n<p>· First aid may be required based on state or facility.</p>\n<p> </p>\n<p> </p>\n<p> </p>\n<p> </p>\n<p> </p>\n<p>We are committed to providing equal  employment opportunities to all applicants for employment regardless of an individual’s characteristics protected by applicable state, federal and local laws.</p>\n<p> </p>\n<p>COFRGE</p>",
