Description
Position Overview Inabia is seeking a dedicated Physical Therapist to join the rehabilitation team at Rehabilitation Center. This position offers the opportunity to work closely with patients recovering from injury, surgery, and chronic conditions in a skilled nursing environment.
The therapist will help patients regain independence through customized rehabilitation plans and evidence-based therapy techniques.
Key Responsibilities Evaluate patients to determine physical limitations, strength deficits, balance issues, and mobility impairments . Design individualized rehabilitation programs aimed at improving strength, endurance, coordination, and functional mobility . Deliver therapy interventions including therapeutic exercise programs, gait training, balance training, transfer training, and manual therapy techniques . Assist patients recovering from orthopedic surgeries, fractures, strokes, neurological disorders, and general deconditioning . Track patient progress and adjust therapy plans to ensure optimal outcomes. Provide patient and caregiver education on exercise techniques, fall prevention strategies, and safe movement practices . Maintain detailed clinical documentation including evaluations, treatment notes, progress reports, and discharge summaries . Collaborate with interdisciplinary healthcare teams to coordinate comprehensive patient care. Ensure therapy services comply with facility standards, state regulations, and therapy documentation guidelines. Requirements Active Texas Physical Therapist License Graduate of accredited Physical Therapy program SNF or rehabilitation experience preferred Schedule & Pay Details 8-hour day shifts
