This position offers the opportunity to work closely with patients recovering from injury, surgery, and chronic conditions in a skilled nursing environment.</p><p>The therapist will help patients regain independence through customized rehabilitation plans and evidence-based therapy techniques.</p><h2>Key Responsibilities</h2><ul><li>Evaluate patients to determine <strong>physical limitations, strength deficits, balance issues, and mobility impairments</strong>.</li><li>Design individualized rehabilitation programs aimed at improving <strong>strength, endurance, coordination, and functional mobility</strong>.</li><li>Deliver therapy interventions including <strong>therapeutic exercise programs, gait training, balance training, transfer training, and manual therapy techniques</strong>.</li><li>Assist patients recovering from <strong>orthopedic surgeries, fractures, strokes, neurological disorders, and general deconditioning</strong>.</li><li>Track patient progress and adjust therapy plans to ensure optimal outcomes.</li><li>Provide patient and caregiver education on <strong>exercise techniques, fall prevention strategies, and safe movement practices</strong>.</li><li>Maintain detailed clinical documentation including <strong>evaluations, treatment notes, progress reports, and discharge summaries</strong>.</li><li>Collaborate with interdisciplinary healthcare teams to coordinate comprehensive patient care.</li><li>Ensure therapy services comply with facility standards, state regulations, and therapy documentation guidelines.</li></ul><h2>Requirements</h2><ul><li>Active <strong>Texas Physical Therapist License</strong></li><li>Graduate of accredited <strong>Physical Therapy program</strong></li><li>SNF or rehabilitation experience preferred</li></ul><h2>Schedule &amp; Pay Details</h2><ul><li>8-hour day shifts</li></ul>",
