    "ExternalDescriptionStr": "<p>Primary Accountability</p>\n<p>The Licensed Clinical Social Worker (LCSW) is responsible for the provision of primary care medical services in a Patient Centered Health Home context, with a focus on access to care.</p>\n<p>Description of Primary Responsibilities:</p>\n<ol>\n <li>&nbsp;Responsible for the direct provision of behavioral health services meeting all established productivity expectations.\n  <ol>\n   <li>Will provide examination of relevant symptoms and history to determine a diagnosis.</li>\n   <li>Expected to document visits accurately utilizing appropriate templates, and record relevant data to attain a diagnosis.</li>\n   <li>Accountable for attaining budgeted visits.&nbsp;</li>\n   <li>Accountable for meeting clinical care measures.</li>\n   <li>Accountable for locking records in the 72 hour window.</li>\n  </ol></li>\n <li>Accountable for providing excellent customer service and care.&nbsp;\n  <ol>\n   <li>Consistently behaves with courtesy when interacting with patients/ family members, support staff, and Network staff.</li>\n   <li>Provider remains aware of wait time during the daily workflow.</li>\n   <li>Responsible for providing the patient with educational materials and resources appropriate to the patient’s health literacy needs.</li>\n  </ol></li>\n <li>Accountable for patient status as measured by relevant clinical quality measures.\n  <ol>\n   <li>Promotes and participates in daily huddles.&nbsp;</li>\n   <li>As an ancillary health team member, participate in managing the care of patients/ families though individual visits and group classes.</li>\n   <li>Responsible for setting and monitoring self- management plans.</li>\n   <li>Responsible for assisting in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.</li>\n   <li>Completes chart and peer review responsibility in a timely manner.</li>\n  </ol></li>\n <li>Assists the patient care team with performance improvement efforts, staff education, and FHCN promotion.\n  <ol>\n   <li>Responsible for assisting the patient care team with performance evaluations and quality improvement.</li>\n   <li>Where relevant, participate in supervision of pre-licensed behavioral health staff.</li>\n   <li>Participates in Network Health Fairs &amp; other community events.&nbsp;</li>\n   <li>Participates in site &amp; provider meetings in a meaningful way.</li>\n  </ol></li>\n <li>Meets Network and credentialing expectations for licensure, and employment.</li>\n <li>Performs other duties as assigned.</li>\n</ol>\n<p>Description of Primary Attributes</p>\n<p>Professional &amp; Technical Knowledge:</p>\n<ol>\n <li>Performance of the duties and responsibilities of the job requires the equivalent of extensive formal training in behavioral health, including an understanding of the application of the theory and practices of the profession to the operations of the organization, usually in the form of a major in Social Work as part of a Master’s Degree.</li>\n</ol>\n<p>Technical Skills:</p>\n<ol>\n <li>Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.&nbsp;</li>\n <li>Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.</li>\n <li>Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.</li>\n</ol>\n<p>Licenses &amp; Certifications: Licensed as Licensed Clinical Social Worker by the state of California.&nbsp;</p>\n<p>Communications Skills:</p>\n<ol>\n <li>Job duties require the compilation and analysis of information prepared in effective written form, including correspondence, reports, articles or other documentation.</li>\n <li>Duties require preparation and execution of presentations to large groups.</li>\n</ol>\n<p>Physical Demands: The physical demands described here in this job description are representative of those that must be met by an employee to successfully perform the essential functions of this position. The employee is frequently required to stand and walk; and occasionally lift and/or move up to 20 pounds.</p>\n<p>$93,882.72 - $150,212.35 &amp;#43; PLUS&nbsp;SIGNIFICANT ADDITIONAL ANNUAL EARNING POTENTIAL&nbsp;through outpatient productivity and quality pay, generous retention payments, shift differential pay, and extra shift pay. &nbsp;Sign-on bonus/relocation assistance, annual CME days and reimbursement also available. &nbsp;Generous health and wellness benefits and retirement packages also offered.</p>",
