    "ExternalDescriptionStr": "<p><strong>Introduction to the Role and Practice</strong></p><p>Centra’s PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care serving adults aged 55 and older in our community. The mission of PACE is to deliver person-centered care in the setting that’s most appropriate for each participant—whether that’s in their own home, with family, in long-term care, or assisted living.<br>PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs.
