The Program Coordinator maintains a continuum of professional and administrative responsibilities related to the coordination and tracking of assessment and evaluation referrals, including regular interaction with the community, families, and referral sources.

Attends mandatory staff meeting and other meetings as assigned
Receives and tracks incoming referrals
Requests and gathers pertinent information and documentation from referral sources
Collaborates with families and team members for efficient scheduling
Maintains evaluators schedule to ensure timely scheduling of appointments
Communicates with families and referral sources regarding the status of referrals
Manages data in the EHR system
Seeks the best use of materials, equipment, and staff to maximize efficiency and effectiveness
Acts as a good steward of Damar and/or client finances
Takes pride and ownership in Damar’s facilities and assigned work areas
Conducts regular file audits to ensure alignment of billable hours, progress notes, invoices, and other relevant reporting documentation
Actively participating in and/or overseeing data collection and the use of data to improve outcomes, practices and service delivery
Maintains documentation that is accurate and legible
Completes and updates mandatory training and other licensure related items in a timely manner
Will adhere to all policies and procedures as defined in the Employee Handbook and Damar Operational Policies and Procedures Manual
Ensures updated and thorough documentation is completed for each assigned independent contractor and client - Documentation is expected to be accurate and legible
What you need to be considered:
High School Diploma or GED required; Bachelor's degree preferred
Minimum of 2 years of experience in case management required
Minimum age of 18 per licensure requirements
Ability to pass and maintain CPR Certification
Must meet and maintain background screening requirements for all Damar Programs as detailed in policy 1.HR.02
Pre-Employment and Employee Background Checks
Valid Indiana driver’s license and driving record that meets eligibility requirements of Damar’s insurance carrier (for all positions requiring driving)
Successful completion of pre-employment physical required
