This position involves comprehensive assessment, planning, implementation, and evaluation of individualized care plans for patients across various healthcare settings. The Case Manager RN acts as a liaison between patients, families, healthcare providers, and insurance companies to facilitate effective communication and resource allocation. By monitoring patient progress and addressing barriers to care, the Case Manager RN helps reduce hospital readmissions and promotes efficient use of healthcare services.
