Conducting biopsychosocial assessments to determine appropriate level of care for the patient. Collaborating with outpatient providers. Understand and present acuity, rationale for continued stay, treatment goals and discharge criteria. Maintain accurate and complete documentation. Provide support to the team when needed. Maintain ongoing communication with the treatment team. When needed, obtain current information on each patient’s clinical progress from various sources and present complete and appropriate clinical information to managed care companies.
