Description

Job Title: Resource Coordinator Department: Health Homes and Integrated Care Reports To: Director Effective Date: 10/14/16 FLSA: Exempt ____________________________________________________________________ Summary The Resource Coordinator" is responsible for overseeing and facilitating access to all of the services necessary for an individual to stay healthy, prevent hospitalizations and maintain stability. The Resource Coordinator is ultimately responsible for the overall provision and coordination of services to assigned caseload. ______________________________________________________________________ Essential Functions: ● Complete initial and ongoing needs assessments (Child and Adolescent Needs and Strengths; CANS) to determine the individual’s most appropriate level of care management. ● Oversee the client’s Individualized Plan of Care. This includes, but is not limited to, coordination of: a) the enrollee’s provision of services as determined by their acuity level, b) adherence to treatment recommendations; c) monitoring and evaluating patient’s needs, including prevention, wellness, medical, mental health, care transitions, and social and community services where appropriate. ● Meet client contact requirements (keeping in mind that caseloads may be a mixture of various acuity levels): Resource Coordinators serving children will be required to have some face-face visits on a consistent schedule as per the mandates of their acuity level (high, medium, or low). ● Fulfill resource coordination documentation requirements in a timely and accurate manner by effectively utilizing designated Medicaid Analytics Performance Portal; (MAPP) and Electronic Health Records (EHRs) as needed. ● Advocate for clients within agency and external service providers. ● Educate the child/caregiver on care of chronic conditions, immunization, screening and other preventive interventions. ● Help clients to obtain and maintain public benefits necessary to gain health care services, including Medicaid and cash assistance eligibility, Social Security, SNAP, housing, legal services, employment and training supports, and others. ● Communicate and share information with the individual, their families and other caregivers with appropriate consideration for language, literacy and cultural preferences. ● Conduct care planning meetings/conferences and serve as an interdisciplinary team member to coordinate comprehensive and holistic care. ● Identify available community-based resources and actively manage appropriate referrals, access, engagement, follow-up and coordination of services. ● Interact with community and other Child Center of NY programs in diverse ways in an effort to enroll new clients in programs. ● Perform other related duties, as necessary. Supervisory Responsibilities: None. Minimum Qualifications: Education and/or credential requirements are determined by children’s acuity level and require one or more of the following: ● Bachelors of Science, Psychology or Education with two years’ experience. ● Registered Nurse or Licensed Practical Nurse with two years’ experience. ● A Master’s Degree in related field (License preferred). a) ● Relevant expertise and experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare and/or other systems as well as those receiving preventive services. b) ● Experience providing service coordination and information, linkages, and referrals for community-based services. c) ● Resource Coordinators serving high acuity enrollees will be required to have demonstrated knowledge and understanding of the needs of such children and their families as evidenced by additional years of experience, education, or training. d) ● Resource Coordinators assigned to children who have medical fragility must have extensive experience in coordinating their care. Qualifications Preferred: ● Bilingual in one or more of the following: Spanish, Mandarin, Cantonese, Bengali and others, may be required depending on client needs. ● Excellent communication (written and verbal) and organizational skills. ● Passion for serving children and families with special needs. ● Demonstrated ability to engage hard to reach and resistant clients and to explain the service delivery model of care management. ● Ability to understand all services offered at The Child Center of NY and communicate those details and benefits of these services to current and prospective clients. Job Behavioral Expectations: ● Maintain regular consistent and professional attendance, punctuality, personal appearance, and adherence to relevant health & safety procedures. ● Safe and successful performance, including meeting productivity standards. ● Maintain an understanding of the principles, methods and processes needed to perform the job and enhance skills. Attend staff meetings, seminars and in-service training as assigned. ● Demonstrate the ability to complete work in an acceptable manner by the time and/or date established with accuracy, neatness and thoroughness and minimal amount of errors. ● Proven self-management abilities, including meeting deadlines, ensuring compliance with agency policy and procedures, and overseeing complete and timely maintenance of agency records, in accordance with contractual requirements. ● Maintain established departmental policies and procedures including maintain strict confidentiality and adherence to all HIPAA guidelines at all times. Physical demands and work environment: The physical demands and work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Physical demands: While performing the duties of this job, the employee is occasionally required to walk, sit for long period of time; use hands to finger, handle, or feel objects, tools, or control; reach with hands and arms; balance; stoop; talk or hear. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus. Work environment: Work may be performed in the field or in an office environment with normal heating and cooling. Work may be performed at the home of the client. Work may be performed during weekday evenings and/or weekends.
