Description Stamford Health has an opportunity for a full-time Clinical Pharmacy Specialist in Stamford, CT! The Clinical Pharmacy Specialist will work onsite Monday - Friday 8-4/9-5 in Stamford. He/she works collaboratively with physicians, APPs, nurses, pharmacists, and IT informatics to provide comprehensive care to a diverse patient population within the hematology oncology space. He/she shall possess expertise and specialized knowledge in antineoplastic agent selection, management, and monitoring to promote safe, evidence-based, efficient, and cost-effective care. Responsibilities: Improves patient centered care and medication outcomes by supporting the needs of the oncology team, oncology providers, our patients, and all members of the organization, such as engagement in quality initiatives. Ensures safe medication practices are delivered by focusing on treatment appropriateness, concomitant disease states, effectiveness, safety, cost awareness, and adherence to treatments. Serves as the Oncology Subject Matter Expert (SME) to provide insights to formulary selection, clinical pathways, medication policies, and guidelines. Works in collaboration with the Pharmacy Informatics Specialist as the oncology SME in partnership with the IT pharmacy analysts to address any electronic health record (EHR) upgrades, testing and modifications. Participates in organizational multi-disciplinary, pharmacy and Bennett Cancer Center EHR workgroup(s) to develop, review, and implement new medications and treatment plans to provide safe and effective treatment pathways in the clinical system for oncology providers, nurses, and pharmacists. Performs the final EHR validation as the SME of new medication treatment plans in collaboration with clinical nursing and the provider and/or disease state lead. Reviews all new medication and participates in treatment plan validations. Brings all new medications and treatment plan requests that do not meet criteria to Pharmacy & Therapeutic Committee for approval. Promotes safe practice by minimizing the risk of errors and medication related problems in medication ordering, preparation, and administration for both the outpatient and inpatient settings. Writes, reviews, and manages all new, existing, or modified chemotherapy and biotherapy-related written clinical policies. Ensures new policies are reviewed and approved by Pharmacy & Therapeutics Committee prior to implementation. Ensures treatment regimens align with Dana-Farber clinical pathways and NCCN approved and/or ensures supportive evidence-based documentation is relevant and available to guide clinical staff. Acts as pharmacy liaison to ensure alignment of chemotherapy and biotherapy policies and procedures between Bennett Cancer Center and inpatient pharmacy and nursing services. Performs other related duties as assigned or requested in order to maintain a high level of service. Completes required continuous training and education, including department specific requirements. Demonstrates professional work behavior by following Service Standards and Success factors. Complies with departmental organizational policies and procedures and adheres to external agency requirements. Graduate from an accredited College of Pharmacy Doctor of Pharmacy degree Licensed Pharmacist (or eligible for licensure) in the State of Connecticut Maintains all licensure/registration requirements applicable to the role Possesses knowledge of complex anticancer therapy regimens as well as knowledge in safe prescribing Must possess excellent verbal and written communication skills, excellent time management skills and the ability to effectively interact and collaborate with other members of the healthcare team #RN123

Ensures new policies are reviewed and approved by Pharmacy &amp; Therapeutics Committee prior to implementation.&nbsp;</li><li>Ensures treatment regimens align with Dana-Farber clinical pathways and NCCN approved and/or ensures supportive evidence-based documentation is relevant and available to guide clinical staff.&nbsp;</li><li>Acts as pharmacy liaison to ensure alignment of chemotherapy and biotherapy policies and procedures between Bennett Cancer Center and inpatient pharmacy and nursing services.&nbsp;</li></ol><p>&nbsp;</p><p><strong>Qualifications:</strong></p><ol><li><strong>PGY2 residency in oncology or PGY1 residency with 2 years of clinical experience in oncology practice setting&nbsp;</strong></li><li>Board Certification in Oncology (BCOP), preferred on hire or obtained within 1 year from date of hire.&nbsp;</li><li>Graduate from an accredited College of Pharmacy&nbsp;</li><li>Doctor of Pharmacy degree&nbsp;</li><li>Licensed Pharmacist (or eligible for licensure) in the State of Connecticut&nbsp;</li><li>Maintains all licensure/registration requirements applicable to the role&nbsp;</li><li>Possesses knowledge of complex anticancer therapy regimens as well as knowledge in safe prescribing&nbsp;</li><li>Must possess excellent verbal and written communication skills, excellent time management skills and the ability to effectively interact and collaborate with other members of the healthcare team&nbsp;</li></ol><p>#RN123</p>",
