    "description": "<p><span style=\"font-weight: bold\"><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Trinity Hospice is looking to hire a PRN Social Worker in the Tulsa area! </span></span></p>\n<p><br></p>\n<p><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Job Summary: </span></p>\n<p><span style=\"font-family: arial, sans-serif; font-size: 10pt\">The Social Worker will provide social services under the direction of a Physician, to offer support to our patients and their families. Social work services will be based on the patient’s psychosocial assessment, and the patient and family’s needs. The Social Worker will participate in the coordination of care with the Interdisciplinary Group (IDG) and participate in on-call rotation per Agency needs.</span></p>\n<p><br></p>\n<p><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Essential Functions of the Job:</span></p>\n<ul>\n<li><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Assess patients to identify psychosocial, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals. </span></li>\n<li><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Leverage effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes. </span></li>\n<li><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Meet mandatory continuing education requirements of the agency/licensing board. </span></li>\n<li><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements. </span></li>\n<li><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Promote Agency philosophy and administrative policies to ensure quality of care. </span></li>\n<li><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Attend interdisciplinary team meetings, participate in the patient care planning process and collaborate with the Interdisciplinary Team to promote coordination of patient care.</span></li>\n<li><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Accept clinical assignments that are consistent with education and competence to meet the needs of the patients.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Education, Experience, and Certifications Needed:</span></p>\n<ul>\n<li><span style=\"font-family: arial, sans-serif; font-size: 10pt\">A Masters or Bachelors Degree in Social Work from a State accredited program is required for consideration. </span></li>\n<li><span style=\"font-family: arial, sans-serif; font-size: 10pt\">One (1) year social worker experience working for a hospice program/agency is preferred. </span></li>\n<li><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Working knowledge of community resources. </span></li>\n<li><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Exceptional interpersonal skills. </span></li>\n<li><span style=\"font-family: arial, sans-serif; font-size: 10pt\">Must have a current driver’s license, reliable transportation and valid auto liability insurance.</span></li>\n</ul>",
