Primary Care Advanced Practice Provider (NP/PA) Location: Prime Healthcare Illinois Medical Group – Family Practice, Kankakee, IL Job Type: Full-Time Prime Healthcare is currently seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our Family Practice team at our Primary Care clinic in Kankakee, Illinois . This full-time role offers an excellent opportunity to provide comprehensive outpatient care in a collaborative and community-focused setting.
Responsibilities
• Deliver full-spectrum primary care to patients of all ages• Manage chronic conditions, perform preventative care, and address acute health concerns• Work closely with physicians and clinical staff to ensure coordinated, high-quality care• Maintain accurate and timely documentation in the electronic medical record
Qualifications
