Responsibilities
Your Role:
As the Lead Case Manager, you will be at the forefront of patient care, working closely with hospitalists and community physicians. Your expertise will be vital in assessing patient needs, coordinating care, and ensuring effective communication with health plans. You’ll be instrumental in guiding patients through their healthcare experience, from initial admission to successful discharge, and beyond.
Key Responsibilities:
Patient Engagement: Meet with patients within the first 24 hours of admission to conduct initial assessments and initiate discharge planning. Collaborative Care Coordination: Collaborate daily with hospitalists to review patient care plans, ensuring the appropriateness of services and facilitating necessary transitions. Health Plan Management: Communicate effectively with health plans to manage clinical information and coordinate care. Transition Support: Ensure smooth transitions to Skilled Nursing Facilities, keeping patients and their families informed and supported throughout the process. Comprehensive Care Oversight: Authorize services across various care settings, ensuring timely and appropriate care for our patients. Team Leadership: Conduct documentation audits, manage staffing needs, and provide support to your team, fostering a collaborative and positive work environment. Continuous Improvement: Participate in projects and initiatives that enhance patient care and operational efficiency. A passion for patient advocacy and a commitment to delivering exceptional care. An RN or LVN license from an accredited nursing school, with 3-5 years of acute nursing experience. Strong knowledge of medical and nursing standards of care, paired with excellent communication skills. Qualifications
Qualifications Education: Graduation from an accredited school of nursing required Bachelor of Science degree in Nursing required License/Certifications: Valid CA RN license required Certified Case Management preferred or willing to obtain within 2 years time from start date Current BLS certification from ARC or AHA Experience: 5 years in acute care nursing - Medical Surgical/Tele/ICU / LTACH or 1 year in acute care nursing ( Medical Surgical/Tele/ICU ) with 2 or more year of care management experience (UM or DC Planning). 1 year of previous experience in case management in the acute inpatient or outpatient settings preferred; knowledge of HMO and Medicare rules in inpatient, home health and at the SNF settings preferred Knowledge of workers compensation rules and regulations is highly preferred Knowledge of Epic is highly preferred Cedars-Sinai is an EEO employer.
