Comunilife Inc. is looking to fill a Case Manager position for its Woodhull I Residence, Brooklyn location. The Case Manager’s main responsibility is to provide comprehensive case management services to assigned clients and their family members. The Case Manager is responsible for full coordination of services for all clients assigned; ensure that all assessments and integrated Individual Recovery Plans are completed and all required/needed services are rendered or arranged based on integrated Individual Recovery Plans' specifications. Responsible for a minimum of a monthly home visit. This position offers a generous benefit package including health insurance, dental, vision, dependent care, paid time off, 401(k), tuition assistance program and more.
Areas Of Responsibilities
Ensure that each client is receiving appropriate and effective case management services.
Conduct comprehensive assessments and develop comprehensive integrated Individual Recovery Plans that identify mutually agreed upon goals and objectives.
Conduct face-to-face contacts (home visits/office visits) as specified in the integrated Individual Recovery Plan(s); with a minimum of one home visit per month and one contact per week.
Provide assistance in accessing primary medical care, mental health, and substance abuse as needed; assistance with adherence to medication regiments; teach activities for daily living; nutritional services; recreation programs; assistance in employment assessments, training, and private sector placement services; prevention with positives; and other necessary services to clients to increase their capacity for independent living.
Conduct reassessments and review integrated Individual Recovery Plans as needed on a quarterly basis to ensure that services are appropriate for the clients’ current needs.
Ensures that each client has an annual psychosocial.
Provide crisis intervention to clients and their families as needed.
Maintain ongoing awareness of clients’ medical conditions so that services are appropriate to changing needs.
Arrange support and home health care services as necessary, making referrals to medical, home care, mental health, and/or substance abuse services
Ensure that clients pay the portion of their rent (30% of household income), telephone and utility bills.
Provide counseling and orientation regarding tenant’s rights and responsibilities to maintain and retain housing.
Assist clients in moving from hospitals, nursing homes, or other locations into their apartments, and, provide transportation when necessary.
In collaboration with the vocational counselor and Entitlement Specialist provide advocacy & referrals to educational, training, and/or employment programs and/or job placement in the private sector.
Assist clients in locating alternative housing options, when a client’s health so requires.
Provide and coordinate all supportive services as needed.
Collaborate with all providers within and outside of Comunilife involved with the client/family.
Maintain regular personal contact, developing a trusting working relationship with clients in order to monitor physical and emotional capacity for independent living.
Maintain and complete required documentation including all contacts made with the client and any contact on their behalf.
Participate in weekly supervision.
Submit a weekly schedule of field visits.
Participate in mandated training workshops on evidence-based practices, seminars, and case presentations as well as meetings with peers and supervisors.
Qualifications
Education and Experience Requirement(s)
Bachelor's Degree in Social Work, Public Administration preferred. Associates degree with at least 4 years of related experience or High School degree with at least 6 years of case management experience may be accepted. Individual, family, and group counseling experience required, particularly with mentally ill populations. Preference is given to persons with experience in Congregate and/or Scatter Site Housing Programs, bi-lingual, bi-cultural in Spanish, a plus.

This position offers a generous benefit package including health insurance, dental, vision, dependent care, paid time off, 401(k), tuition assistance program and more. \\r\\n\\r\\n \\r\\n\\r\\nAreas Of Responsibilities\\r\\n\\r\\n\\r\\n\\t\\r\\n\\tEnsure that each client is receiving appropriate and effective case management services.\\r\\n\\t\\r\\n\\t\\r\\n\\tConduct comprehensive assessments and develop comprehensive integrated Individual Recovery Plans that identify mutually agreed upon goals and objectives.\\r\\n\\t\\r\\n\\t\\r\\n\\tConduct face-to-face contacts (home visits/office visits) as specified in the integrated Individual Recovery Plan(s); with a minimum of one home visit per month and one contact per week.\\r\\n\\t\\r\\n\\t\\r\\n\\tProvide assistance in accessing primary medical care, mental health, and substance abuse as needed; assistance with adherence to medication regiments; teach activities for daily living; nutritional services; recreation programs; assistance in employment assessments, training, and private sector placement services; prevention with positives; and other necessary services to clients to increase their capacity for independent living. \\r\\n\\t\\r\\n\\t\\r\\n\\tConduct reassessments and review integrated Individual Recovery Plans as needed on a quarterly basis to ensure that services are appropriate for the clients’ current needs.\\r\\n\\t\\r\\n\\t\\r\\n\\tEnsures that each client has an annual psychosocial.\\r\\n\\t\\r\\n\\t\\r\\n\\tProvide crisis intervention to clients and their families as needed.\\r\\n\\t\\r\\n\\t\\r\\n\\tMaintain ongoing awareness of clients’ medical conditions so that services are appropriate to changing needs.\\r\\n\\t\\r\\n\\t\\r\\n\\tArrange support and home health care services as necessary, making referrals to medical, home care, mental health, and/or substance abuse services\\r\\n\\t\\r\\n\\t\\r\\n\\tEnsure that clients pay the portion of their rent (30% of household income), telephone and utility bills.\\r\\n\\t\\r\\n\\t\\r\\n\\tProvide counseling and orientation regarding tenant’s rights and responsibilities to maintain and retain housing.\\r\\n\\t\\r\\n\\t\\r\\n\\tAssist clients in moving from hospitals, nursing homes, or other locations into their apartments, and, provide transportation when necessary.\\r\\n\\t\\r\\n\\t\\r\\n\\t In collaboration with the vocational counselor and Entitlement Specialist provide advocacy & referrals to educational, training, and/or employment programs and/or job placement in the private sector.\\r\\n\\t\\r\\n\\t\\r\\n\\tAssist clients in locating alternative housing options, when a client’s health so requires.\\r\\n\\t\\r\\n\\t\\r\\n\\tProvide and coordinate all supportive services as needed.\\r\\n\\t\\r\\n\\t\\r\\n\\tCollaborate with all providers within and outside of Comunilife involved with the client/family.\\r\\n\\t\\r\\n\\t\\r\\n\\tMaintain regular personal contact, developing a trusting working relationship with clients in order to monitor physical and emotional capacity for independent living.\\r\\n\\t\\r\\n\\t\\r\\n\\tMaintain and complete required documentation including all contacts made with the client and any contact on their behalf.\\r\\n\\t\\r\\n\\t\\r\\n\\tParticipate in weekly supervision.\\r\\n\\t\\r\\n\\t\\r\\n\\tSubmit a weekly schedule of field visits.\\r\\n\\t\\r\\n\\t\\r\\n\\tParticipate in mandated training workshops on evidence-based practices, seminars, and case presentations as well as meetings with peers and supervisors.\\r\\n\\t\\r\\n\\r\\n\",\"employmentType\":\"FULL_TIME\",\"hiringOrganization\":{\"@type\":\"Organization\",\"name\":\"Comunilife Inc\",\"logo\":\"https://www.paycomonline.net/v4/ats/web.php/application/style/logo?clientkey=E9BEC8DADD6898D5616AA25B1FDADE8A\"},\"jobLocation\":{\"@type\":\"Place\",\"address\":{\"streetAddress\":\"179 Throop Avenue\",\"addressLocality\":\"Brooklyn\",\"addressRegion\":\"NY\",\"postalCode\":11206,\"addressCountry\":\"USA\"}},\"qualifications\":\"Education and Experience Requirement(s)\\r\\n\\r\\n \\r\\n\\r\\nBachelor's Degree in Social Work, Public Administration preferred.
