Responsible for a minimum of a monthly home visit.&nbsp;<strong><em>This position offers a generous benefit package including health insurance, dental, vision, dependent care, paid time off, 401(k), tuition assistance program and more.&nbsp;</em></strong></p>\r\n\r\n<p>&nbsp;</p>\r\n\r\n<p><strong><u>Areas Of Responsibilities</u></strong></p>\r\n\r\n<ul>\r\n\t<li>\r\n\t<p>Ensure that each client is receiving appropriate and effective case management services.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Conduct comprehensive assessments and develop comprehensive integrated Individual Recovery Plans that identify mutually agreed upon goals and objectives.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Conduct face-to-face contacts (home visits/office visits) as specified in the integrated Individual Recovery Plan(s); with a minimum of one home visit per month and one contact per week.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Provide assistance in accessing primary medical care, mental health, and substance abuse as needed; assistance with adherence to medication regiments; teach activities for daily living; nutritional services; recreation programs; assistance in employment assessments, training, and private sector placement services; prevention with positives; and other necessary services to clients to increase their capacity for independent living.&nbsp;</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Conduct reassessments and review integrated Individual Recovery Plans as needed on a quarterly basis to ensure that services are appropriate for the clients&rsquo; current needs.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Ensures that each client has an annual psychosocial.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Provide crisis intervention to clients and their families as needed.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Maintain ongoing awareness of clients&rsquo; medical conditions so that services are appropriate to changing needs.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Arrange support and home health care services as necessary, making referrals to medical, home care, mental health, and/or substance abuse services</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Ensure that clients pay the portion of their rent (30% of household income), telephone and utility bills.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Provide counseling and orientation regarding tenant&rsquo;s rights and responsibilities to maintain and retain housing.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Assist clients in moving from hospitals, nursing homes, or other locations into their apartments, and, provide transportation when necessary.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>&nbsp;In collaboration with the vocational counselor and Entitlement Specialist provide advocacy &amp; referrals to educational, training, and/or employment programs and/or job placement in the private sector.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Assist clients in locating alternative housing options, when a client&rsquo;s health so requires.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Provide and coordinate all supportive services as needed.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Collaborate with all providers within and outside of Comunilife involved with the client/family.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Maintain regular personal contact, developing a trusting working relationship with clients in order to monitor physical and emotional capacity for independent living.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Maintain and complete required documentation including all contacts made with the client and any contact on their behalf.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Participate in weekly supervision.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Submit a weekly schedule of field visits.</p>\r\n\t</li>\r\n\t<li>\r\n\t<p>Participate in mandated training workshops on evidence-based practices, seminars, and case presentations as well as meetings with peers and supervisors.</p>\r\n\t</li>\r\n</ul>\r\n",
