Oversight includes making patient and direct care staff assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed and assuring the development, implementation and updates of the individualized plan of care.\n<h2>Responsibilities</h2>\n<strong>PRINCIPAL DUTIES AND RESPONSIBILITIES(*Essential Functions)</strong>  \n<li>Assists with the orientation process of new employees. Conducts supervisory visits of field staff members.</li> \n<li>Completes review of open and closed charts quarterly, and as needed.</li> \n<li>Reviews with manager and staff.</li> \n<li>Utilizes findings to formulate educational improvement plans with staff.</li> \n<li>Develops and implements performance improvement projects annually in collaborations with the manager.</li> \n<li>Assists in annual review of policies and procedures, and as needed establishment and revision of policies and procedures.</li> \n<li>Collaborates with manager for OASIS integrity, education, OASIS scrubbing reports, and review of payer requested charts.</li> \n<li>Collaborates with Home Health multidisciplinary staff, referral sources, patients and caregivers to promote quality care.</li> \n<li>Collaborates with Administrator/Manager to represent Home Care in the community and in hospital activities.</li> \n<li>Seeks opportunities for self-development through networking, peer collaborations, involvement in professional organizations, and other related education offerings.</li> \n<li>Performs other duties as needed and/or assigned.</li> \n<h2>Qualifications</h2>\n<strong>JOB SPECIFICATIONS(Minimum Requirements)</strong> \n<ul> \n <strong>KNOWLEDGE, SKILLS, AND ABILITIES</strong> \n <li>Must have excellent clinical coordination, interpersonal skills both verbal and written.</li> \n <li>Must be knowledgeable of home health services and regulations.</li> \n <li>Must be able to work in a self-directed and collaborative environment.</li> \n <li>Must exhibit strong organizational, documentation and teaching skills.</li> \n <li>Applicant must be currently enrolled in school for BSN</li> \n</ul> \n<strong>EDUCATION</strong> \n<ul> \n <li>Associates Nursing Required</li> \n <li>Bachelors Nursing Preferred</li> \n <li>Two (2) years clinical experience in a Home Health setting.</li> \n <li>2 Home Healthcare Required</li> \n <li>Licensed Registered Nurse � Indiana</li> \n</ul> \n<strong>STANDARDS OF BEHAVIOR</strong> Meets the Standards of Behavior as outlined in Personnel Policy and Procedure #1, Employee Relations Code. \n<strong>CONFIDENTIALITY/HIPAA/CORPORATE COMPLIANCE</strong> Demonstrates knowledge of procedures for protecting and maintaining security, confidentiality and integrity of employee, patient, family, organizational and other medical information.
