YoloCares’ clinical team includes registered nurses, hospice aides, social workers, spiritual care counselors, nurse practitioners, and physicians who support patients and families through every stage of the hospice journey. The team is grounded in clinical excellence, mutual respect, and thoughtful coordination, with a shared commitment to dignity, comfort, and individualized care.</p><p><br></p><p><strong>Position Summary</strong></p><p>The RN – Night Shift reports to the Clinical Manager and is responsible for providing direct patient care under the supervision of the Director or designee. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.</p><p><br></p><p><strong>Key Responsibilities</strong></p><ul><li>Triage phone calls from patients/families.</li><li>Make home visits as staffing issues dictate. Work cooperatively with patients, families, nursing staff, physicians, consultant personnel, and ancillary service providers.</li><li>Report to DPCS/Clinical Manager and day shift nurses with patient updates from night shift with daily reports and EMR charting.</li><li>Work cooperatively with patients, families, nursing staff, physicians, consultant personnel, and ancillary service providers.</li><li>Identify personal education needs and seek formal programs or participate in educational activities or seek other avenues to meet these needs.</li><li>Develop a work style, which demonstrates teamwork, constructive time management as well as effective conflict resolution and problem solving.</li><li>Participate in the evaluation(s) of own performance by identifying strengths and weaknesses and by developing a plan to improve areas of weakness.</li><li>Consistently meet all standards of conduct delineated in the YoloCares Integrity Program: Standards of Conduct: A Guide to Compliance and Ethics.</li></ul><p>Note: The above statements are intended to be a representative summary of the major duties and responsibilities performed by the incumbent of this job.
