We are seeking an experienced&nbsp;<strong>Hospice Clinical Manager, RN</strong> to lead our Austin-based clinical team while providing support to our San Antonio branch when needed. This is an excellent opportunity for a strong hospice professional who thrives in leadership, values patient-centered care, and is passionate about guiding teams through exceptional end-of-life care delivery.</p><p>As the Hospice Clinical Manager, you will play a vital role in ensuring the delivery of high-quality, compassionate hospice services while mentoring and supporting clinical staff. You will collaborate closely with interdisciplinary teams to ensure every patient and family receives dignified, personalized care during life&rsquo;s most important moments.</p><h2>What You&rsquo;ll Do</h2><ul data-spread=\"false\"><li>Lead and oversee hospice clinical operations to ensure quality, continuity, and compliance of patient care services</li><li>Supervise and support interdisciplinary clinical staff, promoting excellence in patient outcomes and compassionate care</li><li>Ensure compliance with state and federal hospice regulations, including Conditions of Participation (COPs)</li><li>Participate in and monitor Quality Assurance and Performance Improvement (QAPI) initiatives</li><li>Collaborate with the Interdisciplinary Group (IDG) to develop and implement individualized patient care plans</li><li>Provide education, coaching, and mentorship to clinical team members</li><li>Promote infection control standards and patient safety practices</li><li>Identify operational or clinical improvement opportunities and communicate recommendations to leadership</li><li>Foster a supportive, team-oriented culture focused on service excellence and patient dignity</li></ul><h2>What We&rsquo;re Looking For</h2><ul data-spread=\"false\"><li>Current RN License</li><li>Valid Driver&rsquo;s License and Auto Insurance</li><li>Current CPR Certification</li><li>Minimum of 2 years of hospice management experience preferred</li><li>HomeCare HomeBase experience preferred</li><li>Strong leadership, communication, and organizational skills</li><li>Passion for hospice care and supporting patients and families during end-of-life transitions</li></ul><h2>Why Join Us?</h2><p>At Choice Health at Home, we are committed to supporting our team members just as passionately as we care for our patients.
