In order to ensure that this population has access to care despite the many day-to-day challenges they face, Unity provides care at a constellation of different sites across the city, and also along a continuum of services, from same day walk-ins, to established primary care, to specialty visits, to inpatient respite care for the medically vulnerable. The Nurse Manager for Healthcare for the Homeless (NM for HCH) has oversight of nursing staffing and systems across all three arms of the HCH program: the CCNV Community Health Center, a dynamic list of Homeless Outreach clinic sites, and an expanding portfolio of Medical Respite sites.&nbsp;</p><p>&nbsp;</p><p><strong>ESSENTIAL FUNCTIONS/</strong><strong>MAJOR DUTIES</strong></p><ul><li>Staffing. Responsible for building a dynamic team of highly flexible, dedicated, autonomous and competent nursing professionals (RNs, LPNs, MAs, CNAs) who assure all nursing staffing needs across the HCH program. In coordination with the HCH tripod, manages daily scheduling for CCNV, Homeless Outreach and Respite, adjusting schedules and re-deploying team members as needed to meet program needs.&nbsp;</li><li>On-site coordination. Rotates through CCNV, Homeless Outreach sites, and Medical Respite sites to evaluate proper implementation, use and coordination of practices and policies for all nursing staff.</li><li>Nursing systems. Designs, implements and monitors systems necessary to the safe, efficient functioning of the program, including supply chain and storage for medical supplies, vaccines and emergency equipment.&nbsp;</li><li>Environmental safety and quality. Maintains a physical environment of care and a team culture that measure and promote safety and quality, from hand hygiene to daily quality controls.</li><li>Compliance. Assures that all nursing services delivered across the HC program are provided in-line with Unity Healthcare Policies &amp; Procedures, and in compliance with all applicable standards and requirements of all applicable regulatory bodies.</li><li>Program monitoring and evaluation. In partnership with the HCH tripod and Unity-wide quality improvement mechanisms, identifies, measures and critically evaluates program process and outcomes measures on a periodic basis.&nbsp;</li><li>HCH program strategic governance. In close collaboration with the HCH tripod, and in close coordination with Unity executive leadership, helps provide strategic governance to the HCH program, including budget development and monitoring, crafting the strategic direction of the HCH program, and generally ensuring that the program meets both quality and sustainability goals.&nbsp;</li></ul><p><strong>&nbsp;</strong></p><p><strong>Additional responsibilities include but are not limited to</strong></p><ul type=\"disc\"><li>People.&nbsp;<ul type=\"circle\"><li>Monitors Nursing: Provider ratios and ensures that program is appropriately staffed, working to fill vacancies and also avoiding staffing overages or inefficiencies.&nbsp;</li><li>Provides new team members with a comprehensive orientation, equipping them with the information they need to succeed in their role.&nbsp;</li><li>Observes team member strengths and weaknesses and provides targeted coaching, mentorship and skills remediation as needed.&nbsp;</li><li>Establishes a format and rhythm for HCH nursing team meetings.&nbsp;</li><li>Evaluates annual performance for all direct reports, including establishment of individual and team goal metrics.&nbsp;</li><li>Collaborates with Health Center Directors, human resources, and medical/clinical leadership to effectively handle disciplinary action concerning clinical issues of nursing staff members.</li><li>Identifies and problem-solves performance issues within the team, including working with HR to develop and document performance improvement plans as needed.&nbsp;</li></ul></li><li>Nursing systems and compliance.<ul type=\"circle\"><li>Oversees medical supply ordering for the HCH program and monitors relevant expenses in line with the approved budget.</li><li>Oversees vaccine ordering, storage and administration for the HCH program sites and monitors relevant budget expenses.&nbsp;</li><li>Oversees the completion of safety and environment of care audits for all HCH care delivery locations according to the number and rhythm defined by Unity nursing leadership.&nbsp;</li><li>Oversees the completion of chart audits for all roles supervised according to the number and rhythm defined by Unity nursing leadership.&nbsp;</li></ul></li><li>Nursing leadership.<ul type=\"circle\"><li>In collaboration with nursing leadership, contributes to the development of standards and processes to support and evaluate nursing and medical assistant education, orientation and competency assessment in the HCH context.&nbsp;</li><li>Contributes to Unity-wide quality improvement efforts and actively promotes a culture of continuous quality improvement.</li></ul></li></ul><ol><li>Serves on committees as designated by the Vice President of Nursing.</li></ol><ul type=\"disc\"><ul type=\"circle\"><li>Provides weekly and monthly reports to Unity executive leadership as requested.&nbsp;</li></ul><li>Program operations and oversight. &nbsp;<ul type=\"circle\"><li>Works closely with the HCH Tripod and Finance to develop and monitor an annual budget for HCH.&nbsp;</li><li>Works closely with the HCH Tripod to monitor and evaluate the performance of the different arms of the HCH program on key process and outcome quality indicators.&nbsp;</li><li>In collaboration with HCD, ensures that necessary contract services are in place for the safe functioning of all sites, including biohazard removal, lab specimen pickup and oxygen delivery.&nbsp;</li><li>Collaborates with the HCH Tripod to organize seasonal vaccine outreach and disease-specific vaccine events as needed.&nbsp;</li><li>Works with Unity&rsquo;s Grants Management team as needed, contributing to application for funding relevant to HCH and ensuring compliance with all grants that include an HCH license</li><li>BLS or CPR certification</li><li>Bachelor&rsquo;s degree in nursing</li><li>Master&rsquo;s degree in nursing, Public Health, and/or other health related field</li><li>Minimum three years of experience as a manager and supervisor&nbsp;</li><li>Minimum 3 years of experience managing public health programs for vulnerable populations</li></ol></li><li>Preferred:&nbsp;<ol><li>Experience working with un-housed populations</li><li>Experience working with substance use disorder and untreated mental illness&nbsp;</li><li>5+ years of clinical FQHC/Ambulatory care experience</li><li>Experience staffing complex programs and managing health care supply chains</li><li>Working knowledge of PCMH, team-based care</li><li>Working
