Description
Overview
Chestnut Health Systems is seeking a Housing Clinician to provide recovery-oriented, trauma-informed mental health services to individuals involved in housing programs. This role supports consumers in achieving stability, wellness, and independence through individualized, consumer-driven services delivered in both office and community-based settings.
The Housing Clinician plays a critical role in assessments, treatment planning, service delivery, and coordination with community and internal partners, particularly those supporting housing stability and substance use recovery.

We provide fully integrated care by combining behavioral health care services with community-based primary care centers and are nationally recognized for research, training, and innovation.
This full-time position will work Monday through Friday from 3:00pm to 11:30pm.
Responsibilities
Clinical Services & Documentation
Complete all required documentation (assessments, treatment/recovery plans, progress notes, and funder paperwork) according to program and agency policies.
Ensure 100% of assessments and treatment/recovery plans are completed before their due dates.
Submit at least 80% of progress notes within 48 hours of providing services.
Direct Consumer Care
Provide services based on each consumer’s individualized treatment, recovery, or safety plan .
Meet assigned productivity expectations .
Complete service pre-authorizations and re-authorizations accurately and on time.
Conduct screenings for new housing referrals when assigned.
Recovery-Oriented Practice
Promote a recovery-focused approach , including trauma-informed care, hope, respect, empowerment, wellness, and connectedness.
Use recovery-oriented language in documentation and interactions with consumers.
Coordination & Advocacy
Connect consumers to community resources , especially substance use treatment and housing supports.
Collaborate with internal programs and natural support systems to improve outcomes.
Team Participation & Leadership
Attend team meetings, consumer staffings, and program development activities .
Support team members and provide backup coverage when needed .
Attend and document 100% of required Rule 132 supervision (if not an LPHA).
Complete required Chestnut trainings and CEUs .
Take on leadership responsibilities as assigned.
Evidence-Based & Community-Based Services
Use evidence-based practices in individual, family, and group therapy.
Provide services in community or natural environments when appropriate.
Professional Standards
Maintain strict confidentiality of all consumer and organizational information.
Complete additional duties as assigned by your supervisor.
Qualifications
High school diploma or equivalent, and five years’ social service or peer experience;
Or Certified Recovery Support Specialist (CRSS) or Certified Peer Recovery Specialist (CPRS);
Or have a bachelor’s degree.
Valid driver’s license and private automobile insurance.
Basic computer proficiency in Outlook, MS Word, EMR.
