This role supports consumers in achieving stability, wellness, and independence through individualized, consumer-driven services delivered in both office and community-based settings.</p>\n<p> </p>\n<p>The Housing Clinician plays a critical role in assessments, treatment planning, service delivery, and coordination with community and internal partners, particularly those supporting housing stability and substance use recovery.</p>\n<p> </p>\n<p>Chestnut Health Systems™ is a leading provider of integrated health care services. We provide fully integrated care by combining behavioral health care services with community-based primary care centers and are nationally recognized for research, training, and innovation.</p>\n<p> </p>\n<p><strong>This full-time position will work Monday through Friday from 3:00pm to 11:30pm.</strong></p>\n<h2>Responsibilities</h2>\n<h2>Clinical Services & Documentation</h2>\n<ul>\n <li><p>Complete all required documentation (assessments, treatment/recovery plans, progress notes, and funder paperwork) according to program and agency policies.</p></li>\n <li><p>Ensure <strong>100% of assessments and treatment/recovery plans</strong> are completed before their due dates.</p></li>\n <li><p>Submit <strong>at least 80% of progress notes within 48 hours</strong> of providing services.</p></li>\n</ul>\n<h2>Direct Consumer Care</h2>\n<ul>\n <li><p>Provide services based on each consumer’s <strong>individualized treatment, recovery, or safety plan</strong>.</p></li>\n <li><p>Meet assigned <strong>productivity expectations</strong>.</p></li>\n <li><p>Complete <strong>service pre-authorizations and re-authorizations</strong> accurately and on time.</p></li>\n <li><p>Conduct <strong>screenings for new housing referrals</strong> when assigned.</p></li>\n</ul>\n<h2>Recovery-Oriented Practice</h2>\n<ul>\n <li><p>Promote a <strong>recovery-focused approach</strong>, including trauma-informed care, hope, respect, empowerment, wellness, and connectedness.</p></li>\n <li><p>Use <strong>recovery-oriented language</strong> in documentation and interactions with consumers.</p></li>\n</ul>\n<h2>Coordination & Advocacy</h2>\n<ul>\n <li><p>Connect consumers to <strong>community resources</strong>, especially substance use treatment and housing supports.</p></li>\n <li><p>Collaborate with <strong>internal programs and natural support systems</strong> to improve outcomes.</p></li>\n</ul>\n<h2>Team Participation & Leadership</h2>\n<ul>\n <li><p>Attend <strong>team meetings, consumer staffings, and program development activities</strong>.</p></li>\n <li><p>Support team members and provide <strong>backup coverage when needed</strong>.</p></li>\n <li><p>Attend and document <strong>100% of required Rule 132 supervision</strong> (if not an LPHA).</p></li>\n <li><p>Complete <strong>required Chestnut trainings and CEUs</strong>.</p></li>\n <li><p>Take on <strong>leadership responsibilities</strong> as assigned.</p></li>\n</ul>\n<h2>Evidence-Based & Community-Based Services</h2>\n<ul>\n <li><p>Use <strong>evidence-based practices</strong> in individual, family, and group therapy.</p></li>\n <li><p>Provide services in <strong>community or natural environments</strong> when appropriate.</p></li>\n</ul>\n<h2>Professional Standards</h2>\n<ul>\n <li><p>Maintain <strong>strict confidentiality</strong> of all consumer and organizational information.</p></li>\n <li><p>Complete <strong>additional duties</strong> as assigned by your supervisor.</p></li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>High school diploma or equivalent, and five years’ social service or peer experience; </li>\n <li>Or Certified Recovery Support Specialist (CRSS) or Certified Peer Recovery Specialist (CPRS); </li>\n <li>Or have a bachelor’s degree.</li>\n <li>Valid driver’s license and private automobile insurance.</li>\n <li>Basic computer proficiency in Outlook, MS Word, EMR.</li>\n</ul>\n<p> </p>\n<p><em>Are you intrigued by this job but don’t meet every single requirement?
