Summary/Objective
The Optometrist is expected to be familiar with the philosophy, goals and objectives of the PCMH model, and work collaboratively with other members of the patient care team to achieve maximal health and vision outcomes for NSHC patients. In this role, the Optometrist will examine NorthShore Health Center patients’ eyes to determine the nature and degree of vision problems or eye disease, and prescribe corrective lenses or procedures according to the scope of their licensing. The Optometrist must meet and maintain credentialing guidelines of NorthShore Health Centers, and for professional licensing with the State of Indiana.
Essential Functions
Assist in developing protocols and procedures for the Optometry Department - reviewed annually.
Responsible for the implementation of patient care QA processes for the optometry department through Monthly continuous quality improvement meetings and chart audits.
Examines eyes of NorthShore Health Center patients and performs various tests to determine visual acuity and perception and to diagnose diseases and other abnormalities such as glaucoma and color blindness.
Prescribes eyeglasses, contact lenses, and other vision aids or therapeutic procedures to correct or conserve vision.
Consults with and refers patients to Ophthalmologist or other health care practitioner(s), if additional medical treatment is determined necessary.
Treats eye problems of children who have trouble reading.
Participates regularly in necessary administrative meetings,
Works with clinic management to develop long range plans for NorthShore Health Center’s Optometry Department.
Ensures compliance with regulations and standards of performance for quality care.
All other duties assigned by the Director of Optometry

Qualifications

Must be at least 18 years of age
Must have reliable transportation
Optometrist License in the State of Indiana
CPR Certification
Valid Indiana Optometric Legend Drug Certificate
Preferred Experience Requirements
Two years of experience working within a healthcare setting.
Minimum Education Requirements
Graduate of an accredited Optometry program
Preferred Education Requirement
Further education & certifications related to specialty

Physical Requirements
While performing the duties of this job, the employee may be required to sit and/or stand for prolonged periods, work longer than eight (8) hour shifts, and to work both day/evening/weekend shifts. Work may include hand dexterity as well as the need to reach, climb, balance, stoop, kneel, crouch, talk, and hear. The employee must occasionally lift and/or move up to 50 lbs. While performing the responsibilities of the job, the employee is required to talk and hear. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to focus.
