Together, our interrelated programs provide a whole-person, whole-family, and whole-life approach that unlocks solutions for a lifetime.</p>\n<h2>Responsibilities</h2>\n<p><strong>Results/Goal Oriented Job Description</strong></p>\n<p> </p>\n<p>Position Purpose: Provide 2 sentences or less summarizing the primary purpose of this position. </p>\n<p> </p>\n<p>The Family Resource Case Worker (FRCW) plays a critical role within the MST (Multisystemic Therapy) team by providing case management support to families. The FRCW engages with and assesses needs of families while collaborating with community-based programs (ex: schools, medical offices, etc) to ensure comprehensive support for families.</p>\n<p> </p>\n<p>Core Responsibilities: Provide a bulleted list of no more than 8 core responsibilities of the role; starting with a verb. Focus on those that provide an indication of the scope and impact of the role. Include information that demonstrates the level of responsibility for material, financial human and information resources.</p>\n<p> </p>\n<p> </p>\n<p>Ensure Successful Family Intake: Conduct intake thorough the completion of initial assessments and gather essential consent forms for new cases, ensuring timely case openings, completeness of information and family understanding of services to achieve a smooth start to treatment.</p>\n<p>Enhance Family Engagement: Provide clear explanations of MST therapy and its benefits to families, achieving high levels of family engagement and participation. Partner with Therapist to develop and implement specific interventions to overcome barriers family face with session pacing and work collaboratively towards treatment goals.</p>\n<p> </p>\n<p>Maximize Resource Access for Families by becoming an expert on community resources and assist families in accessing these resources, including prosocial activities, public benefits, housing, legal, education, and medical services, leading to improved family stability. Collaborate with community resources to support family needs.</p>\n<h2>Qualifications</h2>\n<p><strong>Bachelor's degree in human/social service-related field (including education, Psychology, Social work, or a related subject area)</strong>At least 2 years of relevant experience in child welfare, social services, or related fieldExcellent writing, communication, interpersonal, problem-solving, time management and organizational skillsAbility to work independently and as part of a team to meet deadlines, & prioritize multiple tasks efficiently</p>\n<p>Maintain Accurate Case Records: Efficiently create, manage, and process cases in Evolv, Promis, Connections, and Ebase, in a timely manner ensuring all contacts with families and collaterals are thoroughly documented, up-to-date and accurate.</p>\n<p>Requests and obtain school, medical, dental or other applicable health records on an ongoing basis for all children under the age of 18 or relevant family members to ensure family's needs are being met and that there are up to date records on file to be in compliance with stakeholder regulations.</p>\n<h2>Education Required</h2>Bachelor’s Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution\n<h2>Recruitment Tagline</h2>Hiring Impact, Changing Lives Together\n<h2>Salary Range:</h2>$53,970.64 +( 3,500 Bilingual additive + 3,800 Education additive)",
