Adheres to the plan of care and interim physicians&rsquo; orders. Provides care and/or services in accordance with professional standards, applicable laws and regulations and Agency policies and procedures.&nbsp;</p><p data-gid=\"65140\"><strong data-gid=\"65141\">DUTIES AND RESPONSIBILITIES</strong>:&nbsp;</p><ul data-gid=\"65142\"><li data-gid=\"65143\">&nbsp;Provides skilled nursing care within the nursing scope of practice, the standard of care, and as ordered on the plan of care and in compliance with physician orders.</li><li data-gid=\"65144\">&nbsp;Is supervised by the RN in the patient&rsquo;s home with or without the LPN present at lease every 30 days. LPN must notify supervising RN when 30 day supervision is due for each patient.&nbsp;</li><li data-gid=\"65145\">&nbsp;Documents skilled care and/or services provided in compliance with federal and state laws and regulations, and Agency policies and procedures. Documentation is reviewed by the agency supervising staff.&nbsp;</li><li data-gid=\"65146\">&nbsp;Participates in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, to assure that patient care needs are met.&nbsp;</li><li data-gid=\"65147\">&nbsp;Addresses and supports patient/family cultural practices, as long as such practices do not harm others or interfere with the planned course medical therapy.&nbsp;</li><li data-gid=\"65148\">&nbsp;Promotes and provides patient/family/caregiver education using various verbal and written communication techniques that take into account the patient&rsquo;s/family&rsquo;s cultural, ethnic, and/or personal needs or preferences.&nbsp;</li><li data-gid=\"65149\">Knowledgeable of federal, state regulations, nurse practice acts and Agency&rsquo;s policies and procedures regarding patient care.&nbsp;</li><li data-gid=\"65150\">&nbsp;Communicates with all members of the health care team, including physicians appropriately and in a timely manner. 203 Solutions for Care, Inc.(c) 202&nbsp;</li><li data-gid=\"65151\">Notifies the physician and the RN/Clinical Manager of any changes in the patient&rsquo;s condition and the need to modify the plan of care.&nbsp;</li><li data-gid=\"65152\">&nbsp;Maintains currency of knowledge and skills.&nbsp;</li><li data-gid=\"65153\">&nbsp;Participates in the Agency&rsquo;s Quality Assessment Performance Improvement program.&nbsp;</li><li data-gid=\"65154\">&nbsp;Attends Agency meetings and in-service programs as required. ? Obtains appropriate number of continuing education credits to maintain re-licensure status.&nbsp;</li><li data-gid=\"65155\">&nbsp;Maintains confidentiality of patient and Agency information at all times.&nbsp;</li><li data-gid=\"65156\">&nbsp;May administer pharmaceuticals and or perform special treatments based on special education, experience, licensure/certification requirements.
