This role requires a deep understanding of hospice and the Hospice philosophy, as well as the ability to work collaboratively with families and interdisciplinary teams.</span></span></p>\n\n<p>&nbsp;</p>\n\n<p><strong><span style=\"font-size:11pt\"><span style=\"font-family:Arial,sans-serif\">RESPONSIBILITIES</span></span></strong></p>\n\n<p>&nbsp;</p>\n\n<ul>\n\t<li>\n\t<p><span style=\"font-size:11pt\"><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Perform evaluations for hospice care along with informational visits as needed.</span></span></span></p>\n\t</li>\n\t<li>\n\t<p><span style=\"font-size:11pt\"><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Perform initial assessments and determine hospice eligibility</span></span></span></p>\n\t</li>\n\t<li>\n\t<p><span style=\"font-size:11pt\"><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Establish the initial plan of care in collaboration with physicians and hospice team&nbsp;</span></span></span><span style=\"font-size:11pt\"><span style=\"font-family:Arial,sans-serif\"><span style=\"color:#000000\">Care Coordination</span></span></span></p>\n\t</li>\n\t<li>\n\t<p><span style=\"font-family:Arial,Helvetica,sans-serif\"><span style=\"font-size:14px\">Deliver comprehensive nursing care to patients in hospice settings, focusing on pain management and symptom control.</span></span></p>\n\t</li>\n\t<li>\n\t<p><span style=\"font-family:Arial,Helvetica,sans-serif; font-size:14px\">Identify and evaluate the physical, psychological, social, and spiritual needs of patients and their families.</span></p>\n\t</li>\n\t<li>\n\t<p><span style=\"font-family:Arial,Helvetica,sans-serif\"><span style=\"font-size:14px\">Initiate appropriate interventions and create comprehensive, responsive care plans tailored to each patient&#39;s unique needs.</span></span></p>\n\t</li>\n\t<li>\n\t<p><span style=\"font-family:Arial,Helvetica,sans-serif\"><span style=\"font-size:14px\">Collaborate with the interdisciplinary team (IDT) to ensure seamless delivery of services to patients and families.</span></span></p>\n\t</li>\n\t<li>\n\t<p><span style=\"font-family:Arial,Helvetica,sans-serif\"><span style=\"font-size:14px\">Provide emotional support and education to patients and their families regarding end-of-life issues.</span></span></p>\n\t</li>\n\t<li>\n\t<p><span style=\"font-family:Arial,Helvetica,sans-serif\"><span style=\"font-size:14px\">Provide education to patients, families, and primary caregivers on effective care techniques and best practices.</span></span></p>\n\t</li>\n\t<li>\n\t<p><span style=\"font-family:Arial,Helvetica,sans-serif\"><span style=\"font-size:14px\">Document patient care accurately in electronic medical records (EMR) systems while adhering to HIPAA regulations.</span></span></p>\n\t</li>\n\t<li>\n\t<p><span style=\"font-family:Arial,Helvetica,sans-serif\"><span style=\"font-size:14px\">Participate in case management activities, including discharge planning and coordination of community resources.</span></span></p>\n\t</li>\n\t<li>\n\t<p><span style=\"font-family:Arial,Helvetica,sans-serif\"><span style=\"font-size:14px\">Engage in ongoing education regarding hospice practices, infection control, and patient safety.</span></span></p>\n\t</li>\n</ul>\n\n<p>&nbsp;</p>\n\n<p><span style=\"font-family:Arial,Helvetica,sans-serif\"><span style=\"font-size:14px\"><strong>JOB CONDITIONS</strong></span></span></p>\n\n<ul>\n\t<li>\n\t<p><span style=\"font-family:Arial,Helvetica,sans-serif\"><span style=\"font-size:14px\">May be stressful working with end of life circumstances.</span></span></p>\n\t</li>\n\t<li>\n\t<p><span style=\"font-family:Arial,Helvetica,sans-serif\"><span style=\"font-size:14px\">The ability to access patients&rsquo; homes which may not be routinely wheelchair accessible is required.
