Description Job Summary: The Clinical Case Manager in the Community Support Programs (CSP) is responsible for providing a wide array of culturally sensitive, recovery oriented, trauma sensitive, direct clinical services provided in conjunction with other staff. Services provided by this position follow Chapter DHS 63: Community Support Programs, and include assessment, treatment planning, referrals, case management, supportive psychotherapy, education and support, medication monitoring and deliveries, and assessing mental status and symptomology.  The Clinical Case Manager provides community-based, comprehensive clinical services to individuals with mental health and substance use disorders within an interdisciplinary team. This clinician completes timely and professional documentation . Minimum annual service hours are 1352. Working Environment:  Team-based open office environment, with majority of work occurring in the community.  Hours of operation are M-F 8-4:30. Limited evening, weekend and holiday hours. Occasional after-hours crisis backup. Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions. Duties/Responsibilities: Provision of Clinical Services Performs accurate assessments to evaluate mental health, as well as alcohol and other drug use issues in a culturally sensitive, recovery oriented, trauma sensitive manner. Provides a range of clinical services in conjunction with program staff including outreach, assessment, therapy, treatment planning, referral, and case management, in a  context that is supportive of the consumer’s culture, values, and perspectives. Completes accurate clinical records including intakes, assessments, treatment plans and  updates, progress notes, discharge summaries, and other information, in a timely manner. Program and Journey Participation Attends staff meetings, in-services and other program activities as requested. Participates in Journey committees, trainings and activities as directed and/or approved. Qualifications Education and Experience: Education/Licensure ESSENTIAL Master’s degree in a related behavioral health field. PREFERRED Certification or licensure in applicable behavioral health/substance abuse field or ability to acquire. Experience ESSENTIAL 1 year of experience in related behavioral health field. Commitment to work toward improving cultural sensitivity and inclusion as demonstrated by valuing diversity, recognizing personal limitations in one’s competencies and having the desire to improve. Strong commitment to principles of recovery and to person-centered planning in treatment/service planning and provision. Knowledge of evidence-based practices for treatment of mental health and co-occurring disorders. Experience in mental health and substance abuse assessment, case management, treatment planning and crisis resolution. Ability to communicate effectively (both orally and in writing). Ability to work effectively with an interdisciplinary team with a positive and creative attitude. Ability to work independently and remain organized and efficient. Superior ethics and interpersonal boundaries. Possession of a valid driver's license, a favorable driving record, adequate insurance (per  WI law) and access to a vehicle with willingness to use it for work (i.e., transporting consumers). PREFERRED Bi-lingual, and/or bi-cultural (Spanish/Hmong/Cambodian, American Sign Language). Physical Requirements: Requires driving, sitting, standing, light lifting, bending, and reaching, and the ability to use and operate standard office equipment, including computer, phone, copier, cell phone, etc. Hiring Process: All potential candidates will need to successfully pass the Background Information Disclosure (BID) for entity employees and contractors.  Wisconsin’s Caregivers Law requires background, and criminal checks are required by the Department of Health Services and Wis. Stat. 50.065(6)(c) and Wis. Services provided by this position follow Chapter DHS 63: Community Support Programs, and include assessment, treatment planning, referrals, case management, supportive psychotherapy, education and support, medication monitoring and deliveries, and assessing mental status and symptomology.  The Clinical Case Manager provides community-based, comprehensive clinical services to individuals with mental health and substance use disorders within an interdisciplinary team. This clinician completes timely and professional documentation. Minimum annual service hours are 1352.\\n\\n \\n\\nWorking Environment:  Team-based open office environment, with majority of work occurring in the community.  Hours of operation are M-F 8-4:30. Limited evening, weekend and holiday hours.
