Join us on this meaningful journey where your skills, compassion and dedication will make a remarkable difference in the lives of those we serve.</p><p><strong>How you’ll contribute&nbsp;</strong></p><p>A&nbsp;Mental Health Counselor who excels in this role:</p><ul style=\"list-style-type: disc;\"><li>The Mental Health Provider will be responsible to engage in short term individual therapy with clients to assist them in trying to improve their physical health and emotional well being.</li><li>Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.</li><li><span>&nbsp;</span>Participates in case reviews to evaluate case management and progress.&nbsp;</li><li>Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.&nbsp;</li><li>Assists with discharge planning and processes.&nbsp;</li><li>Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.</li><li><span>&nbsp;</span>Supervises and/or trains new staff, students, and interns. Performs Medicaid screenings as indicated for NHP.</li><li><span>&nbsp;</span>Assists patients with Safety Net applications when needed.
