Description

Responsibilities/Qualifications We are seeking a highly motivated, experienced, and connected Community Liaison to lead referral generation efforts and drive business growth for our home care agency. This is a results-oriented position that demands an individual who understands the industry landscape, has strong existing relationships, and can produce measurable outcomes . This is not an entry-level position. You will be expected to operate independently, take full ownership of your territory, and be accountable for referral metrics that directly impact the company’s profitability. Key Responsibilities: Develop and execute a referral growth strategy targeting hospitals, skilled nursing facilities, assisted living, hospice, physician offices, case managers, and other community partners. Leverage existing relationships to generate qualified referrals for home care services (personal care, companionship, etc.). Maintain a weekly presence (10–15 hours minimum) at assigned referral partners and to strengthen engagement and drive referral flow. Meet or exceed monthly and quarterly referral targets as defined by leadership. Track and report on referral activity, relationship status, and pipeline using CRM or tracking tools. Provide feedback on market trends and competitor activity to inform strategic decisions. Represent the company professionally at community events, health fairs, and networking meetings. Collaborate with intake and care coordination teams to ensure timely onboarding of referred clients.
