The Registered Nurse may at times administer nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Nursing</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\"><strong><u>Duties and Responsibilities:</u></strong></span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">1. Provides services in accordance with the plan of care.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">2. Makes the initial evaluation visit and regularly reevaluates the patient&rsquo;s nursing needs.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">3. Initiates the plan of care and necessary revisions.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">4. Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">5. Coordinates services.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">6. Informs personnel of changes in the condition and needs of the patient.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">7. Counsels the patient and family/significant others in meeting nursing and related needs.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">8. Participates in and presents in-service programs.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">9. Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">10. Determines the amount and type of nursing needed by each individual patient.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">11. Conducts patient care conferences on patients assigned to his/her care.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">12. Participates in utilization review of medical records as assigned.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">13. Completes and submits assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with company defined time frames.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">14. Participates in other professional development activities as required.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\"><u><strong>Knowledge and Skill:</strong></u></span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">1. Must have excellent verbal skills and communication abilities.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">2. Ability to work effectively within a team based environment.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">3. Strong time management and organizational skills.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">4. Must be able to clearly and proficiently communicate with people of all ages, while maintaining an energetic and positive attitude.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">2. Must be able to read and write English, and follow verbal and written instructions.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">3. Minimum of two (2) years in similar position, with retirement living or home-care background preferable.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">4. Ability to exercise initiative and independent judgment</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">5. Ability to work flexible hours including weekends and evenings.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">6. Ability to read, analyze, and interpret human service periodicals, professional journals, technical procedures, and governmental regulations.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">7. Ability to write reports, business correspondence, procedure manuals.</span></p><p style=\"line-height: 1;\"><span style=\"font-size: 14px;\">8.
