    "description": "<p><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Sample Supports is actively seeking an energetic and professional Host Home Provider in the Larimer/Weld County areas to offer a home to a female individual in her 30s with intellectual disabilities, as well as medical and mental health needs. Host Home Providers offer an in-home placement to adults with disabilities of varying needs, allowing each person to live in their own communities with the support they need to stay as independent as possible.</span></p>\n<p><br></p>\n<p><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">This position will serve the individual with her behavioral needs, mental health needs, medical needs, and daily support needs. The individual needs support in general activities of daily living, social and emotional regulation, plus encouragement to participate in needed appointments and services. Our residential team is on-call 24/7 and is there to support you if any needs come up during the placement. You become a part of our integrated care team, and we aim to create placements that fully integrate into both your home and the greater community.</span></p>\n<p><br></p>\n<p><span style=\"font-family: Inter, sans-serif; font-size: 12pt; font-weight: bold\">Sample Supports will provide:</span></p>\n<ul>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">General training on caregiving and the needs of individuals with disabilities<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Trauma-informed and social emotional care responses<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">First-aid, CPR, and general safety trainings for all members of the household<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Person-centered training based on the person in care<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Integrated and adaptive behavior support plans<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Crisis intervention and emergency response planning<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">24/7 on-call support<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Support team of expert clinicians to provide in-home and community-based training as needed<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Up to 30 hours/week of supplemental programming for the individual if needed<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Trained respite providers and proactive respite scheduling support<br></span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-family: Inter, sans-serif; font-size: 12pt; font-weight: bold\">Provider duties and responsibilities:</span></p>\n<ul>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Provide personal assistance, social and emotional support, and other personal care to adults with intellectual and developmental disabilities in a home and community environment<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Support with activities of daily living including shopping and meal preparation, personal care/hygiene routines, and general in-home support tasks as needed<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Community activities based on the needs of the individual – can create a unique activity plan together for their respite stay<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Transportation services as needed – may include transportation to/from scheduled activities and appointments<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Complete documentation for services provided<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Must be competent and willing to perform Non-Violent Crisis Intervention physical managements as trained<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Job responsibilities include reporting any suspected maltreatment, abuse, neglect, or exploitation of an at-risk individual (M/A/N/E).
