Some participants may be being releasing from Correctional Facilities and/or from New York State Psychiatric Centers into the community.</span></p><p style=\"text-align: left;\"><span style=\"font-family: georgia, serif; font-size: 14px;\"><strong id=\"isPasted\">ESSENTIAL FUNCTIONS:&nbsp;</strong></span></p><ul><li id=\"isPasted\" style=\"font-family: georgia, serif; font-size: 14px;\">Knowledge and demonstration of agency core values in day-to-day activities</li><li style=\"font-family: georgia, serif; font-size: 14px;\">Work with individuals, in locating safe and affordable housing in an area of their choice</li><li style=\"font-family: georgia, serif; font-size: 14px;\">Promotes and supports individual movement through program phases, including assisting customer to develop goals, and target activities leading to sustainable housing and successful graduation.&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 14px;\">Purchase individual household furnishings for participant residence</li><li style=\"font-family: georgia, serif; font-size: 14px;\">Assist with obtaining benefits if needed, collect income, diagnostic and complete a rental stipend to determine voucher per month</li><li style=\"font-family: georgia, serif; font-size: 14px;\">Maintain open communication with all treatment and service providers, families, landlords and any other supports or networks the individual is working within the community.</li><li style=\"font-family: georgia, serif; font-size: 14px;\">Assist in outreach and location of the individuals referred</li><li style=\"font-family: georgia, serif; font-size: 14px;\">Responsible for recordkeeping (progress notes) in the Millennium Database and other required paperwork such as service planning, roster maintenance, rental stipends, income and homeless verifications, and any other duties asked by supervisor of supportive housing &amp; meets documentation timeliness standards, as required by the agency.</li><li style=\"font-family: georgia, serif; font-size: 14px;\">Must have own transportation.</li><li style=\"font-family: georgia, serif; font-size: 14px;\">There will be an expectation to see individuals in person twice each month, preferably in their home(s).
