This role includes assessing, diagnosing, and treating patients across the lifespan with acute, chronic, and preventive needs that require non-emergent same-day access. The APP works independently and in collaboration with the care team, aligning clinical practice with organizational values, goals, compliance and performance standards.</p>\n<p> </p>\n<p><span style=\"font-weight: bold\">ESSENTIAL FUNCTIONS:</span></p>\n<p> </p>\n<p><span style=\"font-weight: bold\">Patient Care Delivery</span></p>\n<ul>\n<li>Provide timely evaluation and management of patients presenting for same-day care (walk-ins, urgent appointments, acute issues).</li>\n<li>Conducts comprehensive history taking, physical examinations, and diagnostic assessments to identify health concerns, including urgent and same-day issues.</li>\n<li>Provides evidence-based care across the lifespan (pediatric, adult, and geriatric population).</li>\n<li>Demonstrate flexibility in adapting to a high-volume, fast paced environment with unpredictable patient flow with varying complexity.</li>\n<li>Develops and implements therapeutic treatment plans within scope of practice, considering patients’ physical, emotional, cultural, and financial factors.</li>\n<li>Orders and interprets appropriate diagnostic tests;
