Working closely with the Quality Team Manager, you will play a key role in identifying risks, conducting assessments, supporting regulatory requirements, and driving continuous improvement across the agency.</span></p><p><span style=\"font-size: 16px;\"><strong><span style=\"color: rgb(41, 105, 176);\">Position Summary</span></strong></span></p><p><span style=\"font-size: 16px;\">The LPN Risk Coordinator serves as an integral member of the Quality Team, coordinating and supporting risk management activities designed to enhance patient outcomes, ensure regulatory compliance, and promote organizational excellence. This position focuses on risk assessment and monitoring activities related to Infection Control, Emergency Preparedness, Quality Reporting, and Patient Safety.</span></p><p><span style=\"font-size: 16px;\"><strong><span style=\"color: rgb(41, 105, 176);\">Key Responsibilities</span></strong></span></p><p><span style=\"font-size: 16px;\"><strong>Risk Management &amp; Quality Assurance</strong></span></p><ul><li style=\"line-height: 1.5;\"><span style=\"font-size: 16px;\">Conduct agency-wide risk assessments and assist with the development of mitigation strategies.</span></li><li style=\"line-height: 1.5;\"><span style=\"font-size: 16px;\">Monitor, analyze, and report trends related to patient safety, quality outcomes, and regulatory compliance.</span></li><li style=\"line-height: 1.5;\"><span style=\"font-size: 16px;\">Assist in investigating incidents, adverse events, and patient safety concerns.</span></li><li style=\"line-height: 1.5;\"><span style=\"font-size: 16px;\">Support performance improvement initiatives and corrective action plans.</span></li></ul><p><span style=\"font-size: 16px;\"><strong>Infection Control</strong></span></p><ul><li><span style=\"font-size: 16px;\">Assist in monitoring and evaluating infection prevention and control practices.</span></li><li><span style=\"font-size: 16px;\">Participate in infection surveillance activities and reporting.</span></li><li><span style=\"font-size: 16px;\">Support staff education and compliance with infection control standards and best practices.</span></li></ul><p><span style=\"font-size: 16px;\"><strong>Emergency Preparedness</strong></span></p><ul><li><span style=\"font-size: 16px;\">Coordinate and support emergency preparedness activities and risk assessments.</span></li><li><span style=\"font-size: 16px;\">Assist with emergency management planning, drills, exercises, and documentation.</span></li><li><span style=\"font-size: 16px;\">Help ensure ongoing compliance with state, federal, and accreditation requirements.</span></li></ul><p><span style=\"font-size: 16px;\"><strong>Quality Reporting &amp; Compliance</strong></span></p><ul><li><span style=\"font-size: 16px;\">Collect, track, and analyze quality data and performance indicators.</span></li><li><span style=\"font-size: 16px;\">Assist with regulatory reporting and quality improvement projects.</span></li><li><span style=\"font-size: 16px;\">Support preparation for surveys, audits, and accreditation reviews.</span></li><li><span style=\"font-size: 16px;\">Maintain accurate records and documentation related to quality and risk management activities.</span></li></ul><p><span style=\"font-size: 16px;\"><strong>Team Support</strong></span></p><ul><li><span style=\"font-size: 16px;\">Serve as a backup resource for selected Clinical Education and Compliance functions.</span></li><li><span style=\"font-size: 16px;\">Collaborate with clinical leaders and interdisciplinary teams to promote best practices and organizational goals.</span></li><li><span style=\"font-size: 16px;\">Participate in policy review, development, and implementation efforts.</span></li></ul><p><span style=\"font-size: 16px;\"><strong><span style=\"color: rgb(41, 105, 176);\">Qualifications</span></strong></span></p><p><span style=\"font-size: 16px;\"><strong>Required:</strong></span></p><ul><li><span style=\"font-size: 16px;\">Current Licensed Practical Nurse (LPN) license in good standing.</span></li><li><span style=\"font-size: 16px;\">Minimum of 2 years of clinical nursing experience.</span></li><li><span style=\"font-size: 16px;\">Strong organizational, analytical, and problem-solving skills.</span></li><li><span style=\"font-size: 16px;\">Excellent written and verbal communication abilities.</span></li><li><span style=\"font-size: 16px;\">Proficiency with Microsoft Office and electronic health record systems.</span></li></ul><p><span style=\"font-size: 16px;\"><strong>Preferred:</strong></span></p><ul><li><span style=\"font-size: 16px;\">Experience in home health, hospice, quality improvement, risk management, compliance, infection control, or regulatory oversight.</span></li><li><span style=\"font-size: 16px;\">Knowledge of CMS, state, and accreditation standards.</span></li><li><span style=\"font-size: 16px;\">Experience with data collection, reporting, and performance improvement initiatives.</span></li></ul><p><span style=\"font-size: 16px;
