This role is responsible for delivering high-quality, patient-centered care, including the diagnosis and treatment of acute and chronic conditions, health promotion, and disease prevention. The APRN will float our West Pasco locations to help provide coverage and work collaboratively with physicians and interdisciplinary care teams to ensure continuity of care and improve health outcomes across the communities we serve.</p><h2 data-section-id=\"fukckg\" data-start=\"852\" data-end=\"896\"><strong data-start=\"855\" data-end=\"896\">Essential Duties and Responsibilities</strong></h2><ul data-start=\"897\" data-end=\"2331\"><li data-section-id=\"gboj2c\" data-start=\"897\" data-end=\"1031\">Deliver comprehensive, age-appropriate primary care services within scope of practice in collaboration with supervising physicians</li><li data-section-id=\"5ivsur\" data-start=\"1032\" data-end=\"1165\">Assess, diagnose, and treat acute and chronic medical conditions using evidence-based practices and minimally invasive techniques</li><li data-section-id=\"1sugm67\" data-start=\"1166\" data-end=\"1265\">Educate patients and families on health management, disease prevention, and wellness strategies</li><li data-section-id=\"xyy9hf\" data-start=\"1266\" data-end=\"1377\">Collaborate and communicate effectively with care team members to ensure coordinated, seamless patient care</li><li data-section-id=\"y6rthg\" data-start=\"1378\" data-end=\"1517\">Document all patient encounters accurately and timely in the Electronic Health Record
