This role involves conducting comprehensive assessments, developing individualized care plans, and coordinating care with interdisciplinary teams. The RN Start of Care Nurse ensures that patients receive high-quality, personalized care from the very beginning of their home health journey. The Start of Care Nurse will also perform the Resumption of Care visits. This is a full-time Mon-Fri position with some on-call responsibility.
