HOPE ~ HEALTH ~ HEALING
Seeking a Discharge Planner to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Mitchell Health Center.
Full-Time Employed: 40 hours/week
Mon - Fri
8:30am - 5:30pm
Location: 40 E Mitchell Dr # 100, Phoenix, AZ 85012
(Additional Language Differential Pay Available)
NOW OFFERING AN ADDITIONAL $1.50/hr SMI DIFFERENTIAL
Full Benefits Package, including 401K
Generous PTO/Sick Time (4+ weeks in year 1)
Duties may include:
Provide inpatient visit to BHR within 3 days of admission to offer support and linkage to the Outpatient Clinical Treatment Team.
Participate in individualized service planning that includes the involvement of the BHR to the fullest extent possible.
Engage family/significant other, advocate, and /or designated representative involvement throughout the discharge and treatment process.
Develop and execute a written discharge plan (Care Transition ISP) consistent with the BHRs TERROS Service Plan no later than the third day following a recipient’s admission to an inpatient facility. The discharge plan will be developed in collaboration with the BHR/designated representative/guardian, the TERROS treating Medical Provider, hospital treating physician, assigned outpatient clinician, and family, as appropriate.
Offer or coordinate TERROS psychiatric services as appropriate, within 7 days of hospital discharge, and/or as needed to ensure adequate medication supplies.
The Discharge Planner will maintain contact with the BHR/ involved family to evaluate patient status and efficacy of the service plan for a 30 day period.
Coordinate Primary Care Services.
Facilitate communication between psychiatric and primary care providers.
Offer coordination to an array of services that actively addresses recovery goals, which may include: individual, group, and/or family therapy, outreach, engagement, case management, home based services, family stabilization, medication monitoring, behavior management, skills acquisition training, employment/vocational rehabilitation, and supportive community services, as appropriate.

Qualifications
GED/High school diploma with a minimum of 4 years of behavioral health employment experience, or
An associates degree in the field of behavioral health and 2 years of behavioral health employment experience, or
A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health employment experience, or
A bachelor's degree in the field of behavioral health, or
A master's degree or higher in the field of behavioral health
Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred
Valid AZ Driver License. Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy.
Must have a valid unrestricted Arizona Fingerprint Clearance Card (Level One) or apply for an Arizona Fingerprint Clearance Card within 7 working days of employment with no disqualifying events.
Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions
Must pass a typing test with a minimum speed of 25 WPM.
Must pass background check, TB test and other pre-employment screening.
